摘要
目的:探讨微创钢板固定技术(MIPPO)治疗胫骨干骨折的临床疗效,以及对患者生活质量的影响。方法:选择60例患者随机分为观察组和对照组各30例,对照组采用普通钢板切开解剖复位治疗,观察组采用MIPPO治疗,观察两组患者手术基本情况、术后骨折愈合质量和生活质量。结果:两组患者手术时间相似,差异无统计学意义(P>0.05),但观察组患者切口长度、出血总量、住院时间和愈合时间都优于对照组,差异有统计学意义(P<0.05)。观察组患者临床疗效优12例、良15例、中2例和差1例,对照组临床疗效优8例、良14例、中6例和差2例,差异有统计学意义(P<0.05)。术后6个月,观察组患者SF-36量表各项评分均优于对照组,差异有统计学意义(P<0.05)。结论:MIPPO治疗胫骨干骨折临床疗效好,促进骨折愈合,提高生活质量,是治疗股骨干骨折的有效方法。
Objective: To investigate the clinical efficacy and effects on quality of life of micro-invasive percutaneous plating ( MIPPO ) in treatment of tibial shaft fracture. Methods: Totally 60 eligible patients were selected and randomly divided into observation group and control group, with 30 patients in each group. The control group was treated with common open reduction with anatomical plate, while the observation group trea- ted with MIPPO, and the basic conditions of the surgeries, postoperative qualities of fracture healing, and qualities of life were observed in the patients of the two groups. Results : The patients of the two groups had in- significantly different operative times (P〉0.05) ; but the observation group had significantly better incision length, total blood loss, length of stay, and time to healing than the control group ( all P〈0.05 ). Of the observation group, excellent clinical efficacy was achieved in 12 patients, good clinical efficacy achieved in 15 pa- tients, moderate clinical efficacy achieved in 2 patients, and poor clinical efficacy achieved in 1 patient ; while of the control group, excellent clinical efficacy was achieved in 8 patients, good clinical efficacy achieved in 14 patients, moderate clinical efficacy achieved in 6 patients, and poor clinical efficacy achieved in 2 patients; and the differences were statistically significant (P〈0.05). At 6m after the surgeries, the observation group had significantly better SF-36 scores than the control group (all P〈0.05). Conclusion: MIPPO in treatment of tibial shaft fracture has a good clinical efficacy, can promote fracture healing and elevate quality of life, and is effective.
