共聚焦激光扫描显微镜在诊断色素痣、脂溢性角化症及色素型基底细胞癌的临床应用

Clinical application of confocal laser scanning microscopy in the diagnosis of nevus pigmentosus,seborrheic keratosis and pigmented basal cell carcinoma

目的:对临床疑诊色素痣、脂溢性角化症及色素型基底细胞癌的患者行共聚焦激光扫描显微镜检查,分析其灵敏度与特异度,探讨其在上述3种疾病诊断中的应用价值。方法:对临床疑诊为50例色素痣、42例脂溢性角化症及34例色素型基底细胞癌的患者,行临床初步诊断、共聚焦激光扫描显微镜及病理组织学检查,比较临床诊断与共聚焦激光扫描显微镜诊断灵敏度与特异度。结果:共聚焦激光扫描显微镜在色素痣、脂溢性角化症及色素性基底细胞癌的灵敏度及特异度均高于临床诊断。结论:共聚焦激光扫描显微镜实时、无创及高分辨率,对色素痣、脂溢性角化症及色素型基底细胞癌临床辅助诊断有一定价值,有助于其鉴别诊断。

Objective： Using CKSM to examine the patients with suspected nevus pigmentosus, seborrheic keratosis and pig- mented basal cell carcinoma. The sensitivity and specificity of the CLSM, and the potential application of CLMS in the diag- nosis of these disorders were also assessed. Methods： This study included 50 patients with suspected nevus pigmentosus, 42 patients with suspected seborrheie l〈eratosis, and 34 patients with suspected pigmented basal cell carcinoma. After initial clin- ical diagnosis, all patients were examined with CLSM and histopathology, followed by the comparison of the sensitivity and specificity between clinical diagnosis and CLSM in the diagnosis of these disorders. Result： In comparison to clinical diagno- sis, CLSM exhibited a higher sensitivity and specificity for the diagnosis of nevus pigmentosus, seborrheie keratosis and pig- mented basal cell carcinoma. Conclusion： CLSM is a valuable adjunctive approach in the diagnosis of nevus pigmentosus, seborrheic keratosis and pigmented basal cell carcinoma. It is noninvasive, and can provide high-resolution image in a timely manner. CLSM is also useful for differential diagnosis.
