摘要
目的观察不同剂量地佐辛对腹腔镜手术患者全身麻醉苏醒效果的影响.方法选择宁波市第二医院择期全身麻醉下行腹腔镜手术的患者60例,年龄20-65岁,随机分3组,每组各20例,分别于手术结束前15 min,L组静注地佐辛0.1 mg/kg、H组静注地佐辛0.2 mg/kg、C组静注等容量生理盐水.记录3组患者一般临床资料及苏醒期不良反应的发生情况;以及麻醉诱导前(T_0)、拔管即刻(T_1)、拔管后5 min(T_2)、15 min(T_3)和30 min(T_4)时的平均动脉压(mean arterial pressure,MAP)、心率(heart rate,HR)及口述痛觉评分(verbal rating scales,VRS).结果 L组和H组患者麻醉苏醒期躁动的发生率显著低于C组(P<0.05),H组患者嗜睡和呼吸抑制的发生率明显高于其他两组(P<0 05),恶心呕吐的发生率3组无明显差异(P>0.05).与T_0时比较,C组患者T_1时MAP、HR均明显升高,各组患者T_1-T_4各时点VRS评分均明显升高;与C组比较,L组和H组患者各时点MAP、HR均无明显变化,VRS评分均明显降低(P<0.05).结论腹腔镜手术患者在术毕前静脉注射地佐辛0.1 mg/kg能缓解术后早期疼痛,减少全身麻醉苏醒期躁动发生率,且不良反应更少.
AIM To investigate the effect of different doses of dezocine on the recovery quality from general anesthesia in patients undergoing laparoscopic surgery(LS).METHODS Sixty patients who underwent elective LS under general anesthesia,aged from 20 to65 years,were randomly allocated to three groups to receive intravenous dezocine 0.1mg/ kg(L group,n = 20),dezocine 0.2 mg/ kg(H group,n = 20) or saline solution(C group,n = 20) 15 min before the surgery finished,respectively.The general data of the patients and adverse reactions during the anesthesia recovery period were recorded,and the mean arterial pressure(MAP),heart rate(HR),and verbal rating scales(VRS) were also recorded at different time points as follows:before anesthesia induction(T_0),the time of extubation(T_1),5 min(T_2),15 min(T_3),and 30min(T_4) following extubation.RESULTS The incidence of emergence agitation in the L group and H group were significantly lower than that in the C group(P〈0.05),and the incidence of drowsiness and respiratory depression in the H group was significantly higher than that in the other two groups(P〈0.05),while the incidence of nausea and vomiting had no significant difference in the three groups(P〈0.05).The MAP and HR at T_1 were higher than those at T_0 in the C group(P〈0.05).VRS at other time points were significantly higher than those at T_0 in the three groups.Compared with the C group,the MAP and HR had no obvious changes in the L group and H group,while VRS were significantly lower at T_1-T_4(P〈0.05).CONCLUSION Before the end of surgery,0.1 mg/ kg of intravenous dezocine may relieve early postoperative pain and reduce emergence agitation with a lower rate of adverse reactions in patients undergoing LS.
