Estimation and Analysis on Calculating the Expenditure on Cardio-cerebral Vascular Diseases in Jilin

目的:基于国际最新的卫生费用核算体系2011版(SHA 2011)核算吉林省心脑血管疾病费用。方法:基于SHA2011,从筹资方案、机构流向和服务功能维度核算吉林省心脑血管疾病费用。结果:吉林省心脑血管疾病费用为71.10亿元,占经常性卫生费用的比重为11.26%,占卫生总费用的比重为9.20%,占慢性病经常性卫生费用的比重22.20%;28.53%的心脑血管疾病费用由政府方案筹资,家庭卫生支出的占比偏高,为41.25%;心脑血管疾病费用主要流向医院,基层医疗卫生机构占比较低;心脑血管疾病治疗费用主要集中分布在40~85岁这一人群,其中老年人群占比极高,为70.45%。结论:吉林省心脑血管疾病费用占比高,卫生资源消耗大,同时筹资中家庭个人负担较重,应进一步完善医疗保障政策,切实降低个人疾病负担。吉林省心脑血管疾病费用向医院集中,基层医疗卫生机构比重过低。同时,心脑血管疾病的预防服务费用占比较低,需要合理分配心脑血管疾病费用,使其机构流向和功能分布趋于合理。

Objective： To estimate expenditure on cardio-cerebral vascular diseases in Jilin province based on SHA 2011.Methods： To calculate the expenditure on cardio-cerebral vascular diseases in Jilin by financing scheme, health provider and health functiun based on SHA 2011 framework. Results： The expenditure on cardio-cerebral vascular diseases was 7.11 billion yuan, sharing 11.26% of current health expenditure, 9.20% of total health expenditure（THE） and 22.20% of non-communicable diseases expenditure. 28.53% of the expenditure was financed by government financing schemes, while 41.25% was shouldered by household out-of-pocket（OOP） which was respectively high. The expenditure occurred mainly flowed to hospitals. The primary health care providers shared low-proportion. The medical costs for cardio-cerebral vascular diseases were mainly spent on people aging 40-85, while the elderly accounted tbr 70.45%. Conclusion： Cardio-cerebral vascular diseases took high proportion with great health resuurce con- sumption. Meanwhile, the houshold individual burden was respectively heavy, which needed to implement the medical security policy and decrease individual disease burden. The expenditure on cardio-cerebral vaseular diseases was centered in hospitals. The proportion of primary medical institutions was low, while the pruportion of prevention service expenditure fnr expenditure on cardio-cerebral vascular diseases was low. It needed to reasonably allocate the expenditure for expenditure on cardio-cerebral vascular diseases so as to make the institution flow and function allocation become reasonahle.
