流产后关爱服务模式在首次人工流产未婚女性中的应用效果观察

Effect of post abortion care service model in the first induced abortion of unmarried women

目的:探究PAC服务模式对首次人工流产未婚女性心理状况的影响。方法:选取我院妇产科实行PCA服务模式后首次人工流产未婚女性100例,及未实行PCA服务模式前首次人工流产女性100例。每位患者共需接受两次心理状况测评,首次测评时间为患者首次确定妊娠并确认施行人工流产时,二次测评时间为实施常规无痛人流手术留院观察2h后确定离院前。结果:两组女性第一次SCL-90结果与中国常模比较,精神病性、强迫症状、躯体化、恐怖、焦虑、抑郁、其他(主要为与睡眠质量相关项目)等7个项目均高于全国常模。第二次SCL-90结果强迫症状、躯体化、恐怖、焦虑、抑郁、其他(主要为与睡眠质量相关项目)等6个项目均高于全国常模。PCA组女性两次SCL-90结果比较,强迫症状、躯体化、精神病性、焦虑、抑郁等5个项目均有显著降低。对照组女性两次SCL-90结果比较,强迫症状、精神病性2个项目均有显著降低。结论:常规护理与PCA均可以改善首次人工流产未婚女性心理状况,PCA服务模式较常规护理在改善患者心理状态中有较高的改善作用。

Objectives： To explore the influence of post abortion care（ PAC） service model on the psychological status of unmarried women receiving artificial abortion. Methods： 100 unmarried women received PAC service after abortion and 100 cases unmarried women who didn＇t receive PAC service after abortion were selected. All patients received twice psychological condition assessment. The first assessment was taken before abortion and the second assessment was taken 2 hours after abortion. Results： Psychosis,compulsion symptoms,somatization,phobia,anxiety,depression and other seven items（ mainly for sleep quality） of two groups in the first time of SCL- 90 were higher than Chinese norms. Obsessive- compulsive symptom,physical,terror,anxiety,depression and other six items（ mainly related to sleep quality） in the second SCL- 90 were higher than Chinese norms. In PAC group,obsessive- compulsive symptom,physical,mental illness,anxiety and depression were significantly decreased. In control group,obsessive- compulsive symptoms and mental illness were significantly decreased. Conclusion： Routine nursing and PAC can improve the psychological status of unmarried women after the first time of abortion,and PAC service model has a better effect on improving the psychological state of patients.
