摘要
目的探讨医疗失效模式与效应分析(HFMEA)在医院危险化学品安全管理中的应用效果。方法根据追踪方法学的原理,运用HFMEA方法评估医院危险化学品日常管理流程,进行失效模式及潜在风险因素分析,计算事先风险指数(PRN)值,制定并实施有效防范措施。结果应用HFMEA模式后,医院危险化学品管理的不良事件发生情况均有明显下降;影响危险化学品安全管理的失效模式PRN值由632分降到55分,实施前后比较差异具有统计学意义(P<0.01)。结论 HFMEA方法的应用可有效降低医院危险化学品安全管理中不良事件的发生率,提高医院危险化学品管理的风险意识。
Objective To explore the application effect of health care failure mode and effect analysis（HFMEA） in the safety management on hazardous chemicals in hospital. Methods According to the principle of tracing method, daily management procedure of hospital hazardous chemicals was evaluated with HFMEA and failure modes and potential risk factors were analyzed. Predefined risk index（PRN） was calculated and effective preventive measures were formulated and implemented. Results After the application of HFMEA model, the incidence of hazardous chemicals adverse events was significantly decreased. PRN that affects the failure mode of safety management on hazardous chemicals fell from 632 points to 55 points, with a statistically significant difference（P〈0.01）. Conclusion The application of HFMEA can effectively reduce the incidence of adverse events in the safety management on hazardous chemicals in hospital, and raise the risk awareness of hospital hazardous chemicals management.
