患者胰腺实性假乳头状瘤的CT诊断及鉴别诊断(附8例报告)

Diagnosis and differential diagnosis of solid pseudo-papillary tumor of pancreas by CT(Report of 8 cases)

目的:探讨患者胰腺实性假乳头状瘤的CT表现,提高其术前诊断率。方法:回顾性分析术前均经CT平扫及三期增强检查、并经手术病理证实的8例患者胰腺实性假乳头状瘤的影像资料。结果:2例患者的肿瘤以囊性成分为主,5例实性、囊性成分比例相仿,1例以实性成分为主。患者的实性肿瘤部分CT平扫表现为稍低密度或等密度影,囊性肿瘤部分表现为低密度;增强后,实性肿瘤部分动脉期轻度强化,胰腺实质期及静脉期呈渐进性强化,囊性部分于各期始终未见强化。结论:肿瘤胰腺实性假乳头状瘤的CT表现有一定特征,结合临床,可与其它胰腺肿瘤相鉴别。

Objective To investigate CT imaging features of solid pseudo-papillary tumor of pancreas （ SPTP） , and to im-prove the preoperative diagnosis rate. Methods： Imaging findings of 8 patients with histopathology proven SPTP were retrospectively analyzed. All cases underwent unenhanced plain scanning CT and three phases post-contrast scanning. Results ： In 2 cases, the cystic- component was the main; 5 cases were cystic and solid, and the proportions of solid and cystic components were similar; and 1 case was solid. The solid component showed slightly lower or equal density and the cystic component showed low density on precontrast. Af-ter contrast administration, the solid component was slightly enhanced in the arterial phase and progressively enhanced in the pancreatic parenchymal phase and the venous phase, but the cystic component was not enhanced. Conclusions： Combined with clinical diagno-sis ,the CT features of solid pseudo-papillary tumor of pancreas can be distinguished from other pancreatic tumors.
