米索前列醇在瘢痕子宫无痛人流术中的疗效探讨

Misoprostol Efficacy of Uterine Scar Painless Surgery

目的探讨米索前列醇在瘢痕子宫无痛人流术中应用的临床效果。方法方便选取2014年1月—2015年12月在该院门诊自愿接受无痛人流的120例瘢痕子宫早孕患者为研究对象,随机分为观察组和对照组,各60例,观察组术前给予米索前列醇0.4 mg舌下含服;对照组未给予米索前列醇。两组均进行无痛人流,观察比较两组的宫颈扩张情况、手术时间、术中出血量、人工流产综合症的发生率、术后子宫穿孔并发症、术后子宫收缩幅度、丙泊酚的用量、苏醒时间。结果观察组宫颈总软化率为93.3%,明显高于对照组31.7%,观察组患者手术时间、阴道出血量、人流综合症发生率、子宫穿孔的发生率均明显低于对照组,观察组子宫收缩幅度明显高于对照组,观察组术中丙泊酚用量、麻醉苏醒时间明显低于对照组,差异有统计学意义(P<0.05)。结论舌下含服米索前列醇在瘢痕子宫早孕患者行无痛人流时能软化宫颈,缩短手术时间,减少出血量,减少子宫穿孔,减少丙泊酚的用量,缩短术后苏醒时间,在临床上值得推广。

Objective To investigate the clinical effect of misoprostol in painless abortion uterine scar applications. Methods Convenient selection from January 2014 to December 2015 in our hospital painless voluntarily accepted 120 cases of uterine scar pregnancy patients in this study were randomly divided into observation group and control group, 60 patients,surgery group observed before administering meters misoprostol 0.4 mg sublingual; the control group not given misoprostol.Both groups were painless, two groups were observed incidence of cervical dilatation, operative time, blood loss, abortion syndrome, uterine perforation postoperative complications, postoperative uterine contraction amplitude, propofol the amount of recovery time. Results There were a total of cervical softening rate was 93.3%, significantly higher than 31.7%, the observation group were operative time, vaginal bleeding, the incidence of abortion syndrome, uterine perforation were significantly lower than the control group, the observation group uterus contraction was higher in the observation group intraoperative dose of propofol, anesthesia time was significantly lower than the control group, the difference was statistically significant（P〈0.05）. Conclusion Sublingual misoprostol in patients with scarred uterus during pregnancy painless cervix to soften and shorten surgery time, reduce the amount of bleeding, uterine perforation reduction, reducing the amount of propofol,shorten recovery time after surgery, clinical the worth promoting.
