医院联合社区的延续护理对带T管出院患者护理干预模式效果的研究

Nursing intervention model effect research of continuing nursing care in the hospital joint community in discharge patients with T tube

目的探讨医院联合社区的延续护理对带T管出院患者护理干预模式及其作用。方法随机分组法选择2014年1月-2015年1月在上海市第六人民医院金山分院行胆总管切开探查T管引流术后带T管出院的患者50例为对照组,同法选择2015年2月-2016年2月在同一医院行胆总管切开探查T管引流术后带T管出院的患者50例为观察组,对照组采用常规的出院健康教育和护理指导,观察组在对照组的基础上实施出院后延续护理计划,即医院联合社区对此组病人进行护理干预模式,评价两组患者对留置T管相关知识和技能的掌握情况、自我护理能力、按时复诊及满意度比较、并发症的发生情况。结果观察组患者并发症的发生率明显低于对照组,对留置T管知识掌握情况、自我护理能力、满意度明显高于对照组,差异均有统计学意义（均P〈0.05）。结论延续护理及医院联合社区对胆道术后带T管出院患者的干预模式能降低患者的并发症的发生,提高患者自我护理能力,提高满意度,有利于促进患者早日康复。

Objective To explore the nursing intervention model of continuing nursing care in the hospital joint community in discharge patients with T tube and its effect. Methods 50 cases of patients with T tube drainage after exploration of common bile duct and T tube drainage in Jinshan Branch of Shanghai Sixth People＇s Hospital from January 2014 to January 2015 were randomly selected as control group. And using the same method, 50 cases of patients with T tube drainage after exploration of common bile duct and T tube drainage in Jinshan Branch of Shanghai Sixth People＇s Hospital from February 2015 to February 2016 were randomly selected as observation group. Patients in control group were treated with routine health education and nursing instruction, and patients in observation group were treated with continuing nursing program after discharge on the basis of routine health education and nursing instruction, that was nursing intervention model for patients in the hospital community. T catheter related knowledge and skills, self-care ability and satisfaction, timely referral and the occurrence of complications of the two groups were evaluated. Results The incidence of complications in the observation group was significantly lower than that in the control group, and the knowledge of the indwelling T tube, self-care ability and satisfaction were significantly higher than those of the control group, the difference was statistically significant（P〈0.05）. Conclusion Intervention model of extended nursing and hospital community for patients with T after biliary tract surgery can reduce the incidence of complications, improve patient self-care ability, and improve satisfaction. It is benefit to promote the recovery of patients at an early date.
