角绷带型角膜接触镜对翼状胬肉患者术后临床疗效及舒适性的影响

Clinical curative effect and comfortability of corneal contact lens in pterygium surgery

目的探讨绷带型角膜接触镜对翼状胬肉患者术后临床疗效及舒适性的影响。方法前瞻性病例对照研究。收集2014年10月-2015年10月于我院就诊的翼状胬肉患者,随机分配为两组：戴镜组（42例）和对照组（42例）。两组患者均完成行翼状胬肉切除联合自体角膜缘干细胞移植术,戴镜组在手术结束时放置绷带型角膜接触镜,对照组未放。观察两组患者术后1、3、7d的眼部疼痛症状,角膜上皮愈合情况、并发症及6月后胬肉复发率。将两组数据进行对比并进行统计学分析。结果 （1）观察组在各个观察时间点上角膜上皮的疼痛评分均低于同时间点的对照组,差异均具有统计学意义（P〈0.05）;（2）观察组在各个观察时间点上角膜上皮荧光素染色评分均低于同时间点的对照组,差异具有统计学意义（P〈0.05）。（3）两组6个月后并发症和复发率比较,差异无统计学意义（P〉0.05）。结论绷带型角膜接触镜可以促进翼状胬肉患者角膜创面愈合,明显减轻术后眼部疼痛症状,提高术后的舒适度,具有良好的临床疗效。

Objective To explore the clinical outcome and comfortability of corneal bandage lens（BCL） in pterygium surgery of soft contact lens（SCL） in pterygium surgery. Methods Prospective case control study.Patients diagnosed with pterygium from October 2014 to October 2015 in our hospital were enrolled. Patients were randomly divided into BCL group（42 cases） and control group（42 cases）.All patients were treated by pterygium excision with limbal stem cell transplantation.BCL group wear BCL after surgery.We observed the irritation symptom,pains and corneal epithelial healing on 1,3,7 days postoperatively.The complications and recurrences of pterygium were record 6 months postoperatively.Data between two groups were compared and analyzed. Results（1）The corneal pain score time points of observation group were all lower than those of control group at the same time,and the differences were statistically significant（P 〈 0.05）.（2）The corneal fluorescein staining scores of observation group at each time point were lower than those of control group at the same time point, and the difference was significant（P 〈 0.05）.（3）There was no significant difference between the two groups in the complication and recurrence rate after months（P 〉 0.05）. Conclusions Corneal bandage lens（BCL） in pterygium surgery can improve corneal epithelial healing,and reduce irritation and pains symptom.BCL has better clinical outcome and comfortability.
