腹腔镜下广泛子宫切除+盆腔淋巴结清扫手术配合中存在的问题及探讨

Study on the Problems of Laparoscopic Hysterectomy and Pelvic Lymphadenectomy

目的探讨分析腹腔镜下广泛子宫切除+盆腔淋巴结清扫手术配合中存在的问题及护理措施。方法选取48例在腹腔镜下行"广泛子宫切除+盆腔淋巴结清扫"手术的患者,进行严密的护理干预和配合,对出现的问题进行探讨分析。结果本研究中48例宫颈癌、内膜癌,并行"广泛子宫切除+盆腔淋巴结清扫"术的患者无一例进行中转开腹,腔镜手术均顺利进行,无明显并发症发生。结论在患者行腹腔镜下广泛子宫切除+盆腔淋巴结清扫手术前护理人员要熟悉手术流程,并对于患者在围手术期可能出现的低体温危险、皮下气肿现象气腹相关并发症、皮肤压疮等问题要做到预防性护理和充分的应对措施,只有这样才能做好患者在术中得分护理工作,有助于手术的顺利进行和减少患者术后的并发症。

Objective To investigate the analysis of laparoscopic hysterectomy widely and pelvic lymph node cleaning operation with problems and nursing measures. Methods 48 patients with laparoscopic downward ＂extensive hysterectomy and pelvic lymph node cleaning＂ surgery patients, rigorous nursing intervention and coordination, discusses problems in the analysis. Results 48 cases of cervical cancer, endometrial carcinoma in this study, parallel ＂extensive hysterectomy and pelvic lymph node cleaning＂ no to transfer laparotomy, no patients are endoscopy surgery smoothly, without obvious complications. Conclusion In patients with laparoscopic hysterectomy and pelvic lymph node cleaning before surgery nursing personnel should be familiar with the process of operation, and for patients with perioperative hypothermia danger that may occur, subcutaneous emphysema phenomenon pneumoperitoneum related complications, skin problems such as pressure sores to do preventive care and adequate response, only in this way can make patients in the intraoperative nursing work, help the operation smoothly and reduce the postoperative complications of patients.
