白内障术前OCT检查的临床意义

Clinical significance of OCT examination before cataract surgery

目的探讨光学相干断层扫描(optical coherence tomography,OCT)在白内障术前检查中的临床意义。方法选择OCT检查可见眼底的白内障患者193例217只眼,包括糖尿病史患者40例,高度近视患者10例,术前进行全面详细的眼科常规检查和眼底OCT检查。对比计算裂隙灯用前置镜与OCT检查两种检查方法对眼底病的检出率,了解OCT对白内障术前眼底情况的评估意义。结果术前OCT检出眼底病31例(39只眼),前置镜检出眼底病15例(18只眼)。用配对卡方检验OCT计算出眼底病阳性率与前置镜阳性率在α=0.05水平均具有显著性差异。两种检查方法相比较,OCT检查可以明确白内障患者术结论前眼底情况,预测白内障术后视力,可作为术前常规检查。

Objetive To investigate the clinical significance of optical coherence tomography in the preoperative examination of cataract. Method 217 eyes of 193 patients with cataract were examined by OCT, including 40 cases with diabetes and 10 patients with high myopia, In addition to the comprehensive examinations before the operation, OCT examination of the fundus is also done. Positive detection rate of fundus disease was compared. Result 31 cases（39 eyes） were detected by OCT before operation, and 15cases（18 eyes） were detected by front lens for slit lamp. OCT examination in the positive rate of ocular fundus disease was significantly higher than that of front lens for slit lamp. Conclusion OCT is much more better in the accurate diagnosis of ocular fundus disease and in the postoperative visual acuity prediction, it can be used as a preoperative routine examination.
