复方玄驹胶囊对类风湿关节炎患者Th17细胞/白介素17的影响

The effect of Th17 cell percentage/Interleukin-17 in patients with rheumatoid arthritis after compound Xuanju Capsule therapy

目的:研究复方玄驹胶囊对类风湿关节炎患者外周血Th17细胞的比例及白介素17(IL-17)水平的影响。方法:选择类风湿关节炎患者30例和健康对照30例,类风湿关节炎患者接受复方玄驹胶囊治疗2个月。采用流式细胞仪检测外周血CD4+T淋巴细胞中Th17细胞的比例,ELISA法检测外周血中IL-17的表达水平。结果:治疗前,类风湿关节炎患者Th17细胞亚群的比例为(1.84±0.79)%,显著高于健康对照组(0.62±0.31)%。类风湿关节炎患者血清IL-17水平为387.46±82.21pg/ml,显著高于健康对照组45.36±19.31 pg/ml。复方玄驹治疗1月和2月后,类风湿关节炎患者外周血CD4+细胞中Th17细胞亚群的比例分别为为(1.93±0.58)%和(1.53±0.72)%,与治疗前相比无统计学差异。血清IL-17水平分别为为288.21±49.06pg/ml和122.47±51.26pg/ml,差异有统计学意义。复方玄驹治疗2月后,类风湿关节炎患者临床症状及炎症指标较治疗前改善,差异有统计学意义。结论:Th17细胞及其分泌的IL-17可能在类风湿关节炎的发病中起作用。复方玄驹胶囊能抑制Th17细胞分泌IL-17,但不能降低Th17细胞的比例。

Objective： To explore the effect of compound Xuanju Capsule therapy on circulating Thl7 cell percentage and serum interleukin 17 （IL-17） level in patient with rheumatoid arthritis （RA）. Methods ： 30 patients with RA , who received 2 months compound xuanju capsule therapy , and 30 healthy controls were recruited. The percentage of circulating Th17 cells were determined by fiowcytometry, and serum concentration of IL-17 were determined by ELISA. Results： The percentage of Th17 cells were significantly higher in RA patients（ 1.84 ±0.79）% compared with health controls （0.62 ± 0.31 ） % （ P 〈 0.05 ）. Serum IL-17 level in RA patients （ 387.46 ±82.21 pg/ml） were obviously higher than those in healthy controls（45.36±19.31pg/ml） （ P 〈 0.05 ）. After compound xuanju capsule therapy, no signifieandy differences were found in the percentage of Th17 cells [ 1 month （ 1.93 ± 0.58 ） % and 2 month（ 1.53 ± 0.72 ） % ]. In contrast, serum IL-17 level was signifi- cantly decreased [ 1 month （ 288.21 ± 49.06pg/rnl） and 2 month （ 122.47 ± 51.26pg/ml） ] （ P 〈 0.05 ）. After 2 months therapy, the clinical response and erythrocyte sedimentation rate （ESR） , C-reactive protein（CRP） was significantly improvement than before （P 〈 0.05 ）. Conclusion： Thl7 cells and IL-17 might play an important role in the pathogenesis of RA. Compound xuanju capsule cannot decrease the frequencies but can inhibit the function of Th17 cells.
