老年急性呼吸衰竭患者无创呼吸机辅助治疗价值分析

Value analysis of adjuvant treatment of noninvasive ventilator on elderly patients with acute respiratory failure

目的：探讨老年急性呼吸衰竭患者无创呼吸机辅助治疗的价值。方法选择无创呼吸机辅助通气治疗的老年急性呼吸衰竭患者41例，所有患者均在常规药物治疗的基础上给予无创呼吸机辅助通气治疗，观察无创呼吸机辅助通气治疗的疗效、治疗前后患者的动脉血气指标改善情况和并发症发生情况。结果无创呼吸机辅助通气治疗后，所有患者的临床症状及体征明显改善，未出现患者死亡情况，无创呼吸机辅助通气治疗急性呼吸衰竭的临床有效率为100%；9例患者发生腹胀、误吸、红肿及疼痛等并发症，并发症发生率为21.95%，未出现严重并发症；通气治疗前患者的 pH 值、动脉血氧分压和动脉血二氧化碳分压分别为（7.20±0.05）、（41.8±5.9）mmHg （1 mmHg =0.133 kPa）和（81.8±7.8）mmHg，通气治疗结束后为7.41±0.04、（96.3±6.6）mmHg 和（44.3±8.2）mmHg，差异有统计学意义（t =5.414、6.643、7.445，P ﹤0.01）。结论老年急性呼吸衰竭患者在常规药物治疗的基础上给予无创呼吸机辅助通气治疗，能明显改善患者的临床症状及动脉血气指标。

Objective To investigate the effect of noninvasive ventilator auxiliary treatment on elderly patients with acute respiratory failure. Methods Forty-one elderly patients with acute respiratory failure underwent noninvasive ventilator assisted ventilation therapy were selected. All of the patients were given noninvasive ventilator assisted ventilation therapy on the basis of conventional drug treatment. The therapeutic effects of noninvasive ventilator assisted ventilation,arterial blood gas indexes before and after treatment and complications were observed. Results After noninvasive ventilator assisted ventilation treatment,the clinical symptoms and signs improved,no patients died. The clinical effective rate of non-invasive ventilator assisted ventilation was 100% ;nine patients occurred complications such as abdominal distension,aspiration,swelling and pain,the incidence of complications was 21. 95% ,no severe com-plications occurred. Before treatment,the pH,PaO2 and PaCO2 respectively was（7. 20 ± 0. 05 ）, （41. 8 ± 5. 9）mmHg and（81. 8 ± 7. 8）mmHg（1 mmHg = 0. 133 kPa）,which was（7. 41 ± 0. 04）, （96. 3 ± 6. 6）mmHg and（44. 3 ± 8. 2）mmHg after treatment,there were significant differences（t =5. 414,6. 643,7. 445,P ﹤ 0. 01）. Conclusions Noninvasive ventilator assisted ventilation therapy on the basis of conventional drug treatment for elderly patients with acute respiratory failure,can obviously improve the patientˊs clinical symptoms and arterial blood gas indexes.
