损伤控制外科技术在骨科下肢创伤急救中的应用效果

Application of damage control surgery in the treatment of lower limb trauma in orthopedics

目的：探讨骨科下肢创伤患者应用损伤控制外科技术（DCO）的急救措施及效果。方法选取2015年3月至2016年3月110例骨科下肢创伤患者，根据入院先后顺序随机分为常规组和控制组，每组55例，前者采用常规外科治疗，后者根据损伤控制理论实施3个阶段治疗。结果经过治疗后，控制组临床总有效率显著高于常规组，组间比较差异有统计学意义（P ﹤0.05）；经过治疗后，控制组并发症发生率显著低于常规组，组间比较差异有统计学意义（ P ﹤0.01），控制组平均住院时间显著短于常规组，组间比较差异有统计学意义（ P ﹤0.01）。结论骨科下肢创伤患者采用损伤控制理论技术治疗可以在较短时间内稳定患者的生命体征，并可以有效降低并发症的发生率，进而促进了患者的术后康复，值得临床推广应用。

Objective To investigate the emergency measures and effectiveness of the applica-tion of damage control surgery（DCO）in patients with lower extremity trauma in department of orthope-dics. Methods A total of 110 patients with lower extremity orthopedic trauma from March 2015 to March 2016 were selected,according to the order of admission,the patients were randomly divided into routine treatment group and control group,with 55 cases in each group. The rotine treatment group was given conventional surgical treatment,and the control group was given the three stages of treatment ac-cording to damage control theory. Results After treatment,the clinical total efficiency in routine treat-ment group was significantly higher than that in the control group,the difference was significant（P ﹤0. 05）. After treatment,the complication rate of routine treatment group was significantly lower than that of the control group,the difference was significant（P ﹤ 0. 01）,the average length of hospital stay in rou-tine treatment group was significantly shorter than that in the control group,the difference was significant （P ﹤ 0. 01）. Conclusions By damage control theory techniques,the lower extremity trauma patient’s vital signs are stabilized in a short time,and can effectively reduce the complication incidence,promote the patients recovery. It is worthy of clinical popularization and application.
