抗神经元细胞内抗原抗体相关自身免疫性脑炎临床分析 被引量:11

摘要 自身免疫性脑炎(autoimmune encephalitis,AE)泛指一类针对中枢神经系统抗原产生免疫反应而导致的脑病,多以急性或亚急性癫痫发作、认知障碍及精神症状为主要临床表现.AE抗体可分为两类:一类为抗神经元细胞表面抗原抗体,以抗N-甲基-D-天冬氨酸受体(N-methyl-D-aspartate receptors)抗体为代表,其他如抗甘氨酸受体(glycine receptors)、代谢型谷氨酸受体(metabotropic glutamate receptors)、富亮氨酸胶质瘤失活1蛋白(leucine-rich gliomainactivated protein 1)、y-氨基丁酸B型受体(gammaaminobutyric acid B receptors)、α-氨基-3-羟基-5-甲基-4-异(噁)唑丙酸受体(α-amino-3-hydroxy-5-methyl-4-iso xazolepropionic acid receptor)抗体等;一类为抗神经元细胞内抗原抗体,以抗Hu抗体为代表,包括抗Yo、Ri、Ma2、CV2[即抗坍塌反应调节蛋白5(collapsin response mediatorprotein 5,CRMP5)]、神经元突触囊泡(amphiphysin)、谷氨酸脱羧酶(glutamic acid decarboxylase)抗体等.
