血清 PCT、CRP 和 ALB 对食管癌术后早期吻合口瘘的诊断效果

Value of combined detection of PCT, CRP and ALB in diagnosis of anastomotic fistula post-esophagectomy

目的：评价血清降钙素原（PCT）、C-反应蛋白（CRP）和白蛋白（ALB）水平变化在早期诊断食管癌术后吻合口瘘中的应用价值。方法将2012年6月至2015年6月行食管癌手术的217例患者作为研究对象，按是否发生吻合口瘘分为吻合口瘘组（65例）和非吻合口瘘组（152例）。于术前1 d 及术后第1、4、7天分别检测两组患者血液中的 PCT、CRP 和 ALB 水平，并进行比较。结果两组术前1 d 和术后1 d PCT、CRP 和 ALB 水平比较差异未见统计学意义（P ＞0.05）。吻合口瘘组术后第4天和第7天的 PCT 和 CRP 水平明显高于非吻合口瘘组，术后第4天和第7天 ALB 水平明显低于非吻合口瘘组，差异有统计学意义（P ＜0.05）。结论血清 PCT、CRP 与 ALB对食管癌术后早期吻合口瘘具有临床诊断价值。

Objective To evaluate the diagnostic value of pmcalcitonin（PCT）and C-reactive protein（CRP）and albumin（ALB）on post-esophagectomy anastomotic fistula. Methods A total of 217 patients who underwent esophageal cancer surgery from June 2012 to June 2015 were selected. According to whether occur anastomotic fistula,the 217 patients were divided into anastomotic fistula group （65 cases）and non-anastomotic fistula group（152 cases）. The levels of PCT,CRP and ALB of the patients undergoing surgery were detected and compared on 1 day before surgery,the 1st,4th and 7th day after the surgery. Results On 1 day before surgery,and the 1st after the surgery,the levels of PCT, CRP and ALB of the two groups had no significant difference（P 〉 0. 05）. On the fourth day and seventh day after the surgery,the levels of PCT and CRP in the anastomotic fistula group were higher than those of the non-anastomotic fistula group,and the difference were significant（P 〈 0. 05）. On the fourth day and seventh day after the surgery,the levels of ALB in the anastomotic fistula group were lower than those of the non-anastomotic fistula group,and the differences were significant（P 〈0. 05）. Conclusions PCT,CRP and ALB have clinical diagnosis value for anastomotic fistula post-esophagectomy.
