血清降钙素原检测对心力衰竭患者发生感染的诊断价值

Diagnostic value of serum procalcitonin detection for infection diagnosis in patients with heart failure

目的探究血清降钙素原(procalctonin,PCT)水平异常在对心力衰竭患者发生感染进行检测的诊断价值,为感染患者的治疗提供可靠的依据。方法随机选取丽水市人民医院收治的90例心力衰竭患者,根据是否出现感染分为感染组、非感染组,每组患者45例,分别检测2组患者其体内血清PCT水平,并对比分析2组患者的血清降钙素原水平及检测结果的敏感性、特异性及阳性预测值情况。结果感染组患者治疗前的血清降钙素原水平[(10.34±2.64)ng/ml]明显高于非感染组[(0.23±0.05)ng/ml,P<0.05]。感染组患者治疗后的血清降钙素原水平与非感染组相比差异无统计学意义(P>0.05)。感染组患者诊断结果敏感性(91.11%)、特异性(95.56%)、阳性预测值(88.89%)与非感染组患者(48.89%、55.56%、62.22%)相比明显较高(P<0.05)。结论血清降钙素原检测对心力衰竭患者发生感染的诊断价值显著,感染患者体内该物质水平明显升高,提示患者体内可能出现感染,需积极采取应对措施。PCT诊断敏感性好,具有较高的准确性,阳性预测值相对准确,具有十分重要的临床意义。

Objective To explore the significance of serum procalcitonin（PCT） abnormal level for the diagnosis of infec- tion in patients with heart failure, and provide reliable evidence for the infection treatment. Methods Ninety patients with heart failure were randomly selected from our hospital and were divided into infection group and non-infection group, with 45 cases in each group. Serum PCT levels in two groups were detected and the sensitivity, specificity and positive predic- tive value of PCT detection were compared. Results Serum PCT level in the infection group （ 10.34 ±2.64） ng/ml was significantly higher than non-infected group （ 0.23 ± 0.05 ） ng/ml before the treatment （ P 〈 0.05 ）. There was no signifi- cant difference in serum PCT levels between two groups after the treatment （ P 〉 0.05 ）. Infection diagnosis sensitivity （ 91.11% ）, specificity（ 95.56% ）, positive predictive value （ 88.89% ） in the infected patients were significantly higher than non-infected patients（48.89% ,55.56% ,62.22% ） , P 〈 0.05. Conclusions Serum PCT detection exerts obvious effect for the infection diagnosis of heart failure patients and produces high PCT level in the infected patients, which indi- cates the possibility of infection and needs active measures. This detection has a high sensitivity, accuracy and positive predictive, so it is of great clinical significance.
