经腹与经阴道超声诊断早期异位妊娠的对比研究

Comparative study of early ectopic pregnancy diagnosed by abdominal ultrasound and transvaginal ultrasound

目的对比经腹与经阴道超声扫描2种检查方法在早期异位妊娠诊断上的差异性,以期提高早期异位妊娠诊断的准确率。方法回顾性分析我院2006—2009年96例早期异位妊娠患者,结合临床表现,尿人绒毛膜促性腺激素(β-HCG)及病理结果,对比2种检查方法的准确性。结果术后病理证实96例早期异位妊娠患者中有62例经腹彩色多普勒超声(TAD)诊断为异位妊娠,77例经阴道彩色多普勒超声(TVD)诊断为异位妊娠。TVD诊断异位妊娠的阳性率(80%)明显高于TAD诊断异位妊娠的阳性率(65%),2种诊断方法差异有统计学意义(χ2=5.862,P<0.05)。结合尿-HCG检查结果均可提高2种检查方法的准确率。结论 TVD可为临床提供更多有价值的诊断信息,结合尿-HCG,极大地提高了早期异位妊娠的检查阳性率及诊断符合率。

Objective To compare and analyze transabdominal and transvaginal ultrasound scan in the early diagnosis of ectopic pregnancy, in order to enhance the accuracy of early diagnosis of ectopic pregnancy. Methods Retrospectively analyze 96 cases with suspected ectopic pregnancy of 2006-2009 in our hospital. On the bases of clinical manifestations and surgical results and pathology, compared the accuracy of the two inspection methods. Results Of 96 patients Pathologically confirmed there were 62 cases by transabdominal color Doppler ultrasound（TAD）diagnosed as ectopic pregnancy, 77 cases by transvaginal color Doppler ultrasound（TVD） diagnosed as ectopic preg-nancy. TVD positive diagnosis of ectopic pregnancy rate（80%） was significantly higher than TAD positive diagnosis of ectopic pregnancy rate（65%）, there was a significant difference（ χ^2=5.862, P〈0.05）. Conclusion TVD can provide more valuable clinical diagnostic information, combined with Urine-HCG, it greatly improve the inspection of early ectopic pregnancy rate and the positive diagnostic rate.
