激素替代治疗甲状腺功能减退症的脂代谢水平变化

Changes of lipid metabolism before and after thyroid hormone replacement therapy in patients with pypothyroidism

目的探讨甲状腺功能减退症患者激素替代治疗前后脂代谢水平的变化。方法 188例甲状腺功能减退症患者(观察组)给予左甲状腺素钠片治疗,对比观察组治疗前后的甲状腺激素、血脂水平变化并与100例健康人(对照组)作比较。结果观察组治疗前游离三碘原氨酸(FT3)、游离甲状腺素(FT4)低于对照组,超敏促甲状腺激素(s TSH)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)高于对照组;治疗后观察组FT3、FT4增高,s TSH、TC、TG、LDL-C降低;差异均有统计学意义(P<0.05)。观察组治疗后FT3、FT4、s TSH、TC、TG、LDL-C与对照组比较无显著差异(P>0.05)。结论甲状腺激素替代治疗可有效恢复甲状腺功能减退症患者的甲状腺功能,并显著降低血脂水平。

Objective To understand the changes of lipid metabolism before and after Thyroid hormone replacement therapy in patients with hypothyroidism. Methods 188 patients with hypothyroidism（ chosen as the experimental group） were treated with left thyroid hormone sodium. Then the thyroid hormone and lipid levels of the experimental group after and before the treatment were compared and were also compared with those of 100 healthy people（ chosen as the control group）. Findings The levels of FT3 and FT4 before the treatment in the experimental group were significantly lower and the levels of s TSH,TC,TG,LDL-C significantly higher than those in the control group（ P〈 0. 05）. The levels of FT3,FT4 significantly increased and the levels of s TSH,TC,TG,LDL-C significantly lowered after the treatment in the experimental group（ P〉 0. 05）. No significant difference was found between the two groups after the treatment in the levels of FT3,FT4,s TSH,TC,TG,LDLC（ P 〉0. 05）. Conclusion Thyroid hormone replacement therapy can effectively recover the thyroid function,reduce the lipid level,stable lipid metabolism in patients with hypothyroidism,and improve the quality of life of patients.
