胱抑素C在大肠癌患者血清中的临床意义

Clinical significance of serum cystatin C in patients with colorectal cancer

目的探讨血清胱抑素C(Cys C)在大肠癌患者的表达和临床意义。方法采集广州某三甲医院住院期间治疗的大肠癌患者静脉血78份,其中未转移组45份和转移组33份,离心分离血清,采用日本日立有限公司7600全自动生化分析仪检测血清Cys C的水平并进行分析。结果大肠癌未转移组和转移组血清Cys C分别为(1.36±0.88)mg/L和(1.37±0.71)mg/L,均明显高于正常对照组(0.78±0.10)mg/L,差异均具有统计学意义(P<0.05);大肠癌未转移组、转移组以及正常对照组的高年龄段(61~90岁)血清Cys C水平均相应高于低年龄段(31~60岁),差异均具有统计学意义(P<0.05);未转移组以及转移组接受大肠肿瘤切除手术和辅助化疗7 d后,血清Cys C水平分别为(1.07±0.37)mg/L与(1.04±0.49)mg/L,相对于切除手术之前均有所降低,差异均具有统计学意义(P<0.05)。结论血清胱抑素C在大肠癌患者接受手术前中升高,手术和辅助化疗后降低,作为肿瘤辅助诊断指标具有一定的临床价值。

Objective To explore the clinical value of serum cystatin C in patients with colorectal cancer. Methods The fasting serum samples were collected from the patients with colorectal cancer in a hospital in Guangzhou City,includingthe colorectal cancer patients（78） with metastasis（33） and without metastasis（45）. The serum cystatin C was detected byHitachi 7600 analyzer. Results The levels of cystatin C were（1.36±0.88） mg/L and（1.37±0.71） mg/L in the colorectalcancer without metastasis group and colorectal cancer with metastasis group respectively,and both were significantly higherthan that of the normal control group（0.78±0.10） mg/L（P〈0.05）. In each group,the levels of cystatin C in the high-agedsubgroups（61-90 years） were significantly higher than those in the low-aged subgroups（31-60 years）,and the differenceswere statistically significant（all P〈0.05）. Seven days after surgery and adjuvant chemotherapy,the cystatin C levels of thecolorectal cancer without metastasis group and colorectal cancer with metastasis group were（1.07±0.37） mg/L and（1.04±0.49）mg/L respectively,which were significantly lower than the levels of the two groups before surgery and adjuvant chemotherapy（both P〈0.05）. Conclusion The cystatin C level in colorectal cancer patients is significantly increased before surgery andadjuvant chemotherapy,but is reduced after surgery and adjuvant chemotherapy,which indicates that cystatin C has a certainclinical value as an index of tumor assistant diagnosis.
