摘要
目的探讨截骨矫形手术治疗老年退变性腰椎侧后凸畸形的临床疗效。方法回顾性分析自2011年1月至2015年1月采用经椎弓根椎体截骨(PSO)手术治疗的21例老年退变性腰椎侧后凸畸形患者的临床资料。结果21例患者均顺利完成手术,平均手术时间230min(190-280min).平均出血量1200ml(810-2400ml)。末次随访结果显示VAS评分由(6.8±1.5)分减少至(2.2±1.4)分(P〈0.05)。腰椎前凸(LL)从(5.8±8.2)°增加至(33.1±7.6)°,胸椎后凸(TK)从(22.8±8.4)°增加至(35.2±10.9)°,腰椎侧凸角(LS)从(21.5±9.3)°矫正至(6.8±4.2)°,骨盆倾斜角(PT)从(31.5±9.6)°减少至(24.7±8.3)°,骶骨倾斜角(SS)从(14.8±8.8)°增加至(29.3±10.7)°,矢状面平衡(SVA)从(12.5±7.9)cm恢复至(2.8±3.9)cm,冠状面平衡(CTB)从(3.3±1.9)cm恢复至(1.2±1.0)cm,以上差异均有统计学意义(P〈0.05)。结论经椎弓根椎体截骨矫形可以有效矫正畸形和恢复脊柱冠状面、矢状面平衡,显著改善患者的生活质量,缓解疼痛,是一种安全有效的矫形手术。
Objective To investigate the efficacy of osteotomy in treating elderly degenerative lumbar kyphoscoliosis.Methods A retrospective analysis from January 2011 to January 2015 using transpedicular vertebral osteotomy surgical treatment of 21 cases of elderly patients with degenerative lumbar kyphoscoliosis clinical data. Results All patients were operated successfully.The average operation time was 230min ( 190 to 280min ) and intraoperative blood loss was 1200 ml ( 810 to 2400ml ) .The last follow-up, VAS score decreased from preoperative ( 6.8 ± 1.5 ) to ( 2.2 ± 1.4 ) ( P〈0.05 ) .Lumbar lordosis ( LL ) increased from ( 5.8 ± 8.2 ) ° to ( 33.1 ± 7.6) ° , thoracic kyphosis ( TK ) increased from ( 22.8 ± 8.4 ) ° to ( 35.2 ± 10.9 ) ° , lumbar scoliosis ( L S ) decreased from ( 21.5 ± 9.3 ) ° to ( 6.8 ± 4.2 ) ° , pelvic tilt ( PT ) decreased from ( 31.5 ± 9.6 ) ° to ( 24.7 ± 8.3 ) ° , sacral slope ( SS ) increased from ( 14.8 ± 8.8 ) ° to ( 29.3 ± 10.7 ) ° , sagittal vertical axis ( SVA ) restored from ( 12.5 ± 7.9 ) cm to ( 2.8 ± 3.9 ) cm, and coronal'trunk balance ( CTB ) improved from ( 3.3 ± 1.9 ) cm to ( 1.2 ± 1.0 ) cm, the above difference has statistics significance ( P〈0.05 ) . Conclusion Transpedicular vertebral osteotomy can effectively restore the spinal deformity correction and coronal, sagittal balance, improve patient's quality of life, pain relief, is a safe and effective orthopaedic operation.
