盐酸坦洛新缓释片联合复方金钱草颗粒治疗输尿管双J管留置术后相关症状的效果观察

目的探讨盐酸坦洛新缓释片联合复方金钱草颗粒对输尿管双J管留置术后相关症状的改善作用。方法选择2014年3月至2015年12月输尿管镜碎石术后留置双J管患者110例，随机分为2组。坦洛新缓释片组55例，应用盐酸坦洛新缓释片治疗；联合用药组55例，应用盐酸坦洛新缓释片与复方金钱草颗粒联合治疗。比较两组术后4周拔管时输尿管相关症状问卷（USSQ）、国际前列腺症状评分（IPSS）及膀胱过度活动症症状评分（OABSS）情况。结果所有患者均接受调查并随访4周，联合用药组与坦洛新缓释片组比较，术后4周USSQ中尿路症状评分[（14．8±2．7）VS（24．9±3．7）]，躯体疼痛症状评分[（10．6±1．9）VS（14．9±2．8）]，性生活质量评分[（2．1±O．8）VS（3．5±1．1）]，工作能力评分[（9．8±2．1）VS（12．7±2．4）]；IPSS评分[（8．9±1．6）VS（11．9±1．7）]；OABSS评分[（4．1±1．2）VS（6．5±1．3）]，差异有统计学意义（P〈0．05）。结论留置输尿管双J管影响患者生活质量，盐酸坦洛新缓释片联合复方金钱草颗粒能明显改善相关症状，优于单独服用盐酸坦洛新缓释片，提高患者生活质量。

Objective To evaluate the effect of combination therapy of tamsulosin and complex herba lysmachiae gramules in improving double-J ureteral stent-related symptoms. Methods From March 2014 to December 2015, a total of 110 patients with indwelling double-J ureteral stems after ureteroscopic lithotripsy were enrolled for a 4-week long follow-up.The patients were randomly divided into two groups.Each subject in the tamsulosin group （ n=55 ） was administered tamsulosin 0.2 mg QN. Each subject in the combination group （ n=55 ） was administered tamsulosin 0.2 mg QN and complex herba lysmachiae gramules 6g Tid.All the enrolled patients completed the Ureteral Stent Symptom Questionnaire （ USSQ ） , the International Prostate Symptom Score （ IPSS ） as well as the Overactive Bladder Symptom Score （ OABSS ） twice, once on the day before the surgery and the other on the day after the removal of the stent after 4-week long treatment. Results All enrolled patients completed the study and were followed up for 4 weeks.The scores for the tamsulosin group and combination groupa were as following： the mean urinary symptom index score were （ 14.8 ± 2.7 ） and （ 24.9 ± 3.7 ） , the mean pain index score were （ 10.6 ± 1.9 ） and （ 14.9 ± 2.8 ） , the sex index score were （ 2.1 ± 0.8 ） and （ 3.5 ± 1.1 ） , the work performance index score were （ 9.8 ± 2.1 ） and （ 12.7 ± 2.4 ） , IPSS were （ 8.9 ± 1.6 ） and （ 11.9 _± 1.7 ） and OABSS were （ 4.1 ±1.2 ） and （ 6.5 ± 1.3 ） .As opposed to the tamsulosin group, it was observed that combination group prominently improved and all the scores were statistically significant at p value of 0.05. Conclusion Indwelling double-J ureteral stents have a significant impact on patients＇ quality of life. Combination of tamsulosin with complex herba lysimachiae granules can significantly reduce the related symptoms.The combination therapy outperforms the single use of tamsulosin, improves the quality of life of patients in general.
