多发骨折创伤与单纯骨折患者凝血功能及血小板参数检测对预防血栓相关并发症的意义

Significance of Blood Coagulation Function and Platelet Parameters Detection in the Prevention of Thrombosis Related Complications in Patients with Multiple Fractures and Simple Fractures

目的探讨多发骨折创伤与单纯骨折患者凝血功能及血小板参数检测对预防血栓相关并发症的意义。方法选取2014年2月至2016年2月期间我院骨科收治的骨折创伤患者180例,按骨折类型分为多发骨折组78例与单纯骨折组102例,另选同期来院体检的健康者60名为对照组。检测并比较三组凝血功能指标水平及血小板参数水平的差异。结果多发骨折组、单纯骨折组PT、aPTT、TT、均显著低于对照组(P<0.05);多发骨折组均显著低于单纯骨折组(P<0.01)。多发骨折组、单纯骨折组Fbg、D-D均显著高于对照组(P<0.05);多发骨折组均显著高于单纯骨折组(P<0.01)。多发骨折组、单纯骨折组PLT、PDW、MPV、P-LCR均显著高于对照组(P<0.01);多发骨折组均显著高于单纯骨折组(P<0.01)。结论骨折尤其是多发骨折创伤患者发生血栓相关并发症风险甚高,凝血功能及血小板参数检测对血栓相关并发症的风险性评估与预防指导有一定的临床参考价值。

Objective To investigate the significance of blood coagulation function and platelet parameters detection in the prevention of thrombosis related complications in patients with multiple fractures and patients with simple fractures. Methods A total of 180 patients with fracture trauma admitted to our hospital between February 2014 and February 2016 were divided into multiple fracture group（78 cases）and simple fracture group（102 cases）according to the fracture type. Over the same period,60 healthy people who underwent physical examination in the hospital were selected as control group. The blood coagulation function indicators and platelet parameters were detected and compared between the three groups. Results Multiple fracture group, PT, aPTT, TT, simple fracture group were significantly lower than control group （ P 〈 0.05 ） ; Multiple fractures were significantly lower than simple fracture group （ P 〈 0. 01 ）. Multiple fracture group, Fbg, D- D simple fracture group were significantly higher than that of control group （ P 〈 0.05 ） ; Multiple fractures were significantly higher than the simple fracture group（P 〈0.01 ）;The PLT,PDW,MPV and P- LCR in multiple fracture group and simple fracture group were significantly higher than those in the control group（P 〈 0.01 ） ; PLT, multiple fracture group were significantly higher than simple fracture group （ P 〈 0. 01 ）. Conclusion Fractures, especially in patients with multiple fractures trauma thrombosis related to high risk of complications, function of blood coagulation and platelet parameters detection guidance for risk assessment and prevention of thrombosis related complications has certain clinical reference value.
