缬沙坦对兔耳增生性瘢痕的影响

摘要 目的研究缬沙坦对兔耳增生性瘢痕形成的影响。方法 16只新西兰兔随机分为4组,每只兔耳腹侧制作6个创面。高剂量治疗组:术后第21天开始给予缬沙坦8mg/(kg·d)口服治疗。低剂量治疗组:在术后第21天开始予缬沙坦4mg/(kg·d)口服治疗。阳性对照组:分别在术后第21天和28天时给予皮损内曲安奈德注射治疗。阴性对照组:不给于任何干预。在术后第42天时处死所有兔子,收集所有标本行苏木素-伊红染色和苦味酸天狼星红染色。结果高剂量治疗组的瘢痕增生指数(1.71±0.11)、成纤维细胞数量(438.15±50.01)、Ⅰ型胶原百分比(41.94±2.29)、Ⅲ型胶原百分比(28.83±1.48)均低于阴性对照组(2.02±0.29),(612.08±106.59),(49.51±2.81),(29.80±1.39);低剂量治疗组的Ⅰ型及Ⅲ型胶原百分比(48.24±2.02),(29.05±1.10)均低于阴性对照组;阳性对照组的瘢痕增生指数(1.33±0.13)、成纤维细胞数量(233.08±62.32)、Ⅰ型及Ⅲ型胶原百分比(36.42±2.41),(27.94±2.26)均低于高剂量治疗组。结论缬沙坦可以在一定程度上抑制兔耳瘢痕增生,高剂量治疗效果更佳。 Objective To explore the influence of Valsartan to hypertrophic scar of rabbit ears. Methods 16 New Zealand rabbits were randomly divided into four groups. Six wounds were made on the ventral side of each rabbit. High dose treatment group: Twenty-first days after operation, Valsartan 8mg/(kg · d) was given orally. Low dose treatment group: Twenty-first days after operation, Valsartan 4mg/(kg· d) was given orally. Positive control group: Twenty-first days and 28 days after operation, intralesional injection of triameinolone acetonide for treatment were given. Negative control group: Did not give any intervention. All rabbits were sacrificed forty-second days after operation. Hematoxylin eosin staining and picrosirius red staining were given on all the samples. Results Scar proliferation index, number of fibroblasts, collagen type Ⅰ and collagen type Ⅲ of high dose treatment group were (1.71 ± 0.11), (438.15 ± 50.01), (41.94 ± 2.29), (28.83 ± 1.48) respectively, which were lower than those of negative control group with (2.02 ± 0.29), (612.08 ± 106.59), (49.51 ± 2.81), (29.80 ± 1.39). Collagen type Ⅰ and type Ⅲ of low dose treatment group were (48.24 ± 2.02, 29.05 ± 1.10), which were lower than those of negative control group. Scar proliferation index, number of fibroblasts, collagen type Ⅰ and collagen type Ⅲ of positive control group were (1.33 ± 0.13), (233.08 ± 62.32), (36.42 ± 2.41), (27.94 + 2.26), which were all lower than those of low dose treatment group. Conclusion To some extent, valsartan can inhibit the hypertrophic scar of rabbit ears. High dose treatment effect is better.
