医护一体化管理方式在重型动脉瘤性蛛网膜下腔出血早期介入治疗患者中的应用

The application of doctor-nurse unified management on patients with severe aneurysmal subarachnoid hemorrhage in early period

目的探讨医护一体化管理方式在重型动脉瘤性蛛网膜下腔出血早期介入术后患者中的应用效果。方法选取2014年9月至2016年5月收治的118例重型蛛网膜下腔出血介入术后患者作为观察组,2012年12月至2014年8月收治的93例重型蛛网膜下腔出血介入早期术后患者作为对照组,对照组采用传统的责任制护理模式;观察组采用医护一体化护理模式,包括成立医护一体化协作小组、组织专科护理培训、开展医护一体化查房和术后干预。比较两组患者并发症的发生率、平均住院日、患者满意度及医生对护士的满意度。结果观察组较对照组患者术后并发症发生率下降(P<0.01);平均住院日缩短(P<0.05);护士专科知识及技能提高(P<0.01);患者满意度和医生对护士的满意度均提高(P<0.05)。结论实施医护一体化模式能够降低重型动脉瘤性蛛网膜下腔出血术后患者并发症的发生率,缩短患者平均住院日,提高护士专科水平,提升患者满意度和医生对护士的满意度,值得临床进一步推广和应用。

Objective To explore the application of doctor - nurse unified management on patients with severe aneurysmal subarachnoid hemorrhage in early period. Method Pieked up 118 cases of severe aneurysmal subarachnoid hemorrhage patient from Sep 2014 to May 2016 as the observation group,93 cases of severe aneurysmal subarachnoid hemorrhage patient in early period from Dec 2012 to Aug 2014 as the control group. The observation gwup used the traditional responsibili- ty nursing mode and the control group used the doctor -nurse unified management mode which including established unified cooperation team,organized specialized nursing training,implementing doctor - nurse unified mode and after - operation intervention. The occurrence rate of complication, the average hospitalized dates,sat- isfaction of patient and satisfaction degree of doctor towards nurses of the two groups were compared. Result The occurrence of postoperative complication of obser- vation group was lower than the control group （ P 〈 0.01 ） ,the average hospitalized dates was shorten （ P 〈 0.05 ） ; the specialized knowledge and skills of nurses were improved （ P 〈 0.01 ） ,the satisfaction degree of patients and doctor to nurses were increased （ P 〈 0.05 ）. Conclusion Using doctor - nurse unified mode can decrease the occurrence rate of postoperative complication on severe aneurysmal subarachnoid hemorrhage patients, shorten patients average hospitalized dates, pro- mote the specialized nursing level ,improve the satisfaction degree of patients and doctor to nurse ,which deserves to be spread and used in clinic department.
