摘要
目的探讨胎儿骨骼发育迟缓与孕妇甲状腺功能水平的关系。方法选取该院2013年12月—2016年1月就诊的39例妊娠中期胎儿骨骼发育迟缓患者作为观察组,同时随机选取同期的20例妊娠中期正常发育胎儿作为对照组,彩超检测胎儿肱骨及股骨的长度,同时检测孕母促甲状腺激素(TSH)的水平。待分娩后观察新生儿身长及体重,进行对比研究。结果 1观察组胎儿股骨长度及肱骨长度分别为(3.95±0.23)cm、(3.64±0.18)cm,而对照组分别为(4.56±0.16)cm、(4.18±0.15)cm,二组间差异具有统计学意义(P<0.01)。2观察组孕妇TSH值(6.70±3.66)IU/ml,对照组为(1.78±0.8)IU/ml;二组间差异具有统计学意义(P<0.01)。3孕妇TSH值与股骨长度及肱骨长度的相关性分别为-0.44,-0.37。4出生后新生儿身长及体重,观察组身长为(49.33±1.04)cm,体重为(2 992.82±173.07)kg;而对照组身长为(50.27±0.54)cm,体重为(3 180.00±267.25)kg,二组间差异具有统计学意义(P<0.01)。结论妊娠中期彩超发现胎儿骨骼发育迟缓,结合孕妇TSH水平的检查,可以更好的对胎儿骨骼发育状况进行了解和评估,有助于早期发现妊娠期孕妇甲状腺功能情况,有利于孕妇安全度过妊娠期,促进胎儿更好的生长、发育。为临床早期诊断,治疗提供依据。
Objective To investigate the relationship between fetal skeletal development retardation and the level of thyroid of pregnant women.Methods Thirty-nine pregnant women in Mid pregnancy with fetal skeletal development retardation were selected from December 2013 to January 2015 in author'hospital as observation group,and other twenty cases of normal bone development were randomly selected as control group.The length of the humerus and femur fetus was detected by color ultrasound,and motherhood thyroid-stimulating hormone(TSH)level was also detected.After birth body,length and weight were detected.Results The fetal femur length and the humerus length in the observation group respectively were 3.95±0.23 cm,3.64±0.18 cm,and they were 4.56±0.16 cm,4.18±0.15 cm in the control group,there were statistically significant difference between the two groups(P〈0.01).TSH value of pregnant women in the observation group was 6.70±3.66 Iu/ml,it was 1.78±0.8 Iu/ml in the control group,and there was statistically significant difference between the two groups.The correlation between TSH values and the femur length was-0.44.It was-0.37 between TSH values and the humerus length.After birth the body length in observation group was 49.33±1.04 cm.The weight in observation group was 2992.82±173.07 kg.The body length in control group was 50.27±0.54 cm.The weight in control group was 180.00±267.25 kg.There was statistically significant difference between the two groups(P〈0.01).Conclusions Finding skeletal fetal development retardation in Mid pregnancy with color ultrasound combined with pregnant women clinical TSH level can be used to evaluate the fetal bone development,and detect thyroid function of pregnant women during pregnancy.These results can help pregnant women to safety pass through pregnancy,Help fetal growth and development better.This rusults provide a base for clinical early diagnosis and treatment.
