慢性肝病肝穿活检组织病理学研究

Histopathological Study on Liver Biopsy of Chronic Liver Disease

目的探讨慢性肝病患者肝穿活检组织病理学特征.方法筛选106例符合慢性肝病组织穿刺活检标本,所有标本行HE染色、Masson染色、网状纤维染色及D-PAS特殊染色,并评估肝穿组织炎症活动度及纤维化程度.结果 106例肝穿刺活检组织中检测出慢性肝炎86例,肝炎性早期肝硬化13例,肝炎后肝硬化7例,其中3例合并肝癌.在86例慢性肝炎中,轻度慢性肝炎44例,中度慢性肝炎33例,重度慢性肝炎9例.106例慢性肝病中,G1S0 12例,G1S1 39例,G2S2 26例,G3S3 19例,G4S4 10例.结论肝穿刺病理检查对评估肝组织活动度和肝纤维化分期具有重要价值.

Objective To explore the histopathological features of liver biopsy in chronic liver disease.Method Specimens of 106 cases with chronic liver disease were selected in this study.All specimens underwent HE staining,special staining including Masson,reticular fibers and D-PAS staining in order to evaluate the degrees of liver tissue inflammation and fibrosis.Results Among the 106 cases of liver biopsy,86 cases were chronic hepatitis（CHB）,13 cases were early liver cirrhosis,7 cases were posthepatitic cirrhosis and 3 cases were combined with liver cancer.Among the 86 cases of chronic hepatitis,44 cases were mild chronic hepatitis,33 cases were moderate chronic hepatitis,9 cases were severe chronic hepatitis.Among these 106 cases,12 cases were inflammation grade 1 fibrosis 0（G1S0）,39 cases were grade 1 stage 1（G1S1）,26 cases were grade 2 stage 2（G2S2）,19 cases were grade 3 stage 3（G3S3）,10 cases were grade 4 stage 4（G4S4）.Conclusion The liver puncture pathological detection possesses important valuation for the evaluation of liver tissue inflammation and fibrosis,and can provide important clinical significance for the diagnosis and treatment of chronic liver disease.
