索利那新治疗小儿尿道下裂术后膀胱痉挛的有效性及安全性分析

Analysis on the efficacy and safety of solifenacin in the treatment of blad-der spasm after hypospadias surgery in children

目的研究索利那新治疗小儿尿道下裂术后膀胱痉挛的有效性及安全性。方法以我院2014年1月~2016年8月小儿尿道下裂术后膀胱痉挛患儿90例为研究对象并进行分组。参照组术后服用安慰剂,干预组术后给予索利那新治疗。比较两组患儿用药安全性、膀胱痉挛次数、痉挛持续时间、膀胱冲洗液转清时间、患儿尿管拔除后3 d平均尿量和日均排尿次数、疼痛评分的差异。结果所有患儿无严重不良反应,其中,参照组无口干、腹泻或头晕情况,干预组口干1例,头晕1例,腹泻1例,并发症发生率为6.67%,两组对比无显著差异(P>0.05),干预组较参照组膀胱痉挛次数更少,痉挛持续时间、膀胱冲洗液转清时间更短(P<0.05);干预组较参照组尿管拔除后3 d平均尿量更高,日均排尿次数更少(P<0.05)。两组疼痛评分无显著差异(P>0.05)。结论索利那新治疗小儿尿道下裂术后膀胱痉挛的有效性及安全性高,可有效缓解膀胱痉挛,减少排尿次数,增加排尿量,且无严重不良反应,安全性高,值得推广。

Objective To study the efficacy and safety of solifenacin in the treatment of bladder spasm after hypospa-dias surgery in children. Methods A total of 90 cases of children with bladder spasm after hypospadias surgery in Our hospital from January 2014 to August 2016 were divided into groups. The control group was given placebo postopera- tively. The intervention group was given solifenacin postoperatively. The drug safety was compared between the two groups; frequency of bladder spasm, spasm duration, and the time of bladder flushing fluid to clear were compared be- tween the two groups; the differences of the average urinary volume and the number of daily urination, and pain score 3 days after removal of urinary catheter in children were compared. Results No serious adverse reactions were observed in all children. Among them, the control group was without dry mouth, diarrhea or dizziness. 1 ease of dry mouth, 1 case of dizziness and 1 case of diarrhea were observed in the intervention group, with the incidence rate of complica- tions of 6.67%. There was no significant difference between the two groups（P〉0.05）. The frequency of bladder spasm in the intervention group was less than that in the control group. Spasm duration, and the time of bladder flushing fluid turning to clear were shorter（P〈0.05）; compared with the control group, the average urine volume in the intervention group was higher after the removal of urine catheter, and the number of daily urination was lower（P〈0.05）. There was no significant difference in pain score between the two groups（P〉0.05）. Conclusion Solifenacin is highly effective and safe in the treatment of bladder spasm after hypospadias surgery in children. It can effe.ctively relieve bladder spasm, reduce the. frequency of urination, increase urine Volume, and has no serious adverse reactions. It is safe and worthy to be popularized.
