生物反馈-认知行为联合前庭康复训练治疗慢性主观性头晕的疗效研究

The efficacy of biofeedback behavioral cognitive and vestibular rehabilitation treatment on patients with chronic subjective dizziness

目的探讨生物反馈一认知行为联合前庭康复训练治疗慢性主观性头晕的疗效。方法将60例慢性主观性头晕患者采用随机数字表法分为研究组（盐酸度洛西汀肠溶胶囊联合生物反馈一认知行为治疗及前庭康复训练）30例和对照组（盐酸度洛西汀肠溶胶囊）30例。采用眩晕残障程度评定量表（DHI）、焦虑抑郁量表（HADS）、简明健康调查问卷（SF-36）进行评估。结果在治疗第4、6周末，研究组DHI、HADS量表评分差异有统计学意义[研究组：DHI4周（9．07±3．18）分，6周（4．67±3．08）分，HADS4周（3．17±1．56）分，6周（2．03±1．10）分；对照组：DHI4周（13．20±4．48）分，6周（12．13±3．60）分，HADS4周（4．90±2．40）分，6周（4．57±2．06）分]（均P〈0．05）。SF-36量表评分在生理功能（PF）、生理职能（RP）、躯体疼痛（BP）、总体健康（GH）、活力（VT）、社会功能（SF）、情感职能（RE）以及精神健康（MH）8个方面治疗6周后研究组[分别为（96．17±5．20）分，（95．00±5．72）分，（96．67±4．79）分，（95．17±5．33）分，（95．50±5．14）分，（94．58±9．10）分，（88．90±18．21）分，（96．67±4．59）分]与对照组[分别为（85．33±11．29）分，（82．67±9．07）分，（88．53±8．63）分，（80．83±7．44）分，（88．00±8．47）分，（80．00±14．90）分，（64．44±27．60）分，（81．73±14．04）分]评分差异有统计学意义（均P〈0．05）。结论生物反馈-认知行为治疗联合前庭康复训练治疗慢性主观性头晕，能显著改善患者的头晕症状，以及患者焦虑、抑郁情绪，提高生活质量。

Objective To observe the efficacy of biofeedback behavioral cognitive and vestibular rehabilitation treatment in patients with chronic subjective dizziness（CSD）. Methods 60 patients with chronic subjective dizziness were divided into two groups： study group （30 cases） treated with drug,vestibular rehabilitation and biofeedback behavioral cognitive therapy, and the control group （30 cases） was treated with drug treatment by random number table method, and all the 60 patients in the study were assessed by DHI,HADS and SF-36. Results After four and six weeks of treatment,the DHI and HADS scale scores of the study group were（ 4 weeks：DHI（ 9.07±3.18）, HADS（ 3.17± 1.56） ;6 weeks ： DHI（4.67±3.08）, HADS （2.03± 1.10） ） ,and the scores of the control group were（ 4 weeks ：DHI（ 13.20±4.48）, HADS（4.90±2.40） ;6 weeks ： DHI （ 12.13± 3.60 ）, IIADS （ 4.57 ± 2.06 ） ）, and the differences were statistically significant （ all P〈 0.05 ）.After 6 weeks treatment, SF-36 scores in eight aspects about physiological function, role-physical, bodily pain, general health, vitality, social-functioning, role-emotional, and mental health, in the study group were （ 96.17±5.20）, （95.00±5.72）, （ 96.67±4.79）, （ 95.17±5.33 ）, （ 95.50±5.14）, （ 94.58±9.10）, （ 88.90± 18.21 ）, （96.67±4.59）and the control group were（ 85.33± 11.29）, （82.67±9.07）, （88.53±8.63）, （80.83± 7.44）, （88.00±8.47） , （ 80.00± 14.90） , （ 64.44±27.60）, （ 81.73± 14.04 ） , the differences were statistically significant（P〈0.05 ）.Conclusion The study indicates that vestibular rehabilitation and biofeedback behavioral cognitive therapy can significantly improve symptoms of dizziness, anxiety, depression and quality of life.
