类风湿关节炎患者外周血血浆中TNF-α、IL-1β及IL-10的表达及临床意义

Expressions of TNF-α,IL-1β and IL-10 in Peripheral Blood of Rheumatoid Arthritis Patients and Its Clinical Significance

目的:观察类风湿关节炎(RA)患者血浆中TNF-α、IL-1β及IL-10的表达情况,探讨TNF-α、IL-1β、IL-10与RA疾病活动度指标血浆ESR、CRP水平及DAS28评分的相关关系。方法:30例RA患者和10例健康志愿者,采集相关临床资料,检测C反应蛋白(CRP)及血沉(ESR)等,并进行DAS28评分。ELISA检测血浆中TNF-α、IL-1β及IL-10的表达,分析炎性细胞因子与疾病活动度指标的相关关系。结果:RA活动组患者血浆TNF-α、IL-1β水平明显高于健康对照组(P<0.05),IL-10水平有显著下降(P<0.05)。RA缓解组TNF-α、IL-10水平较健康对照组有明显升高,(P<0.05),IL-1β无明显变化(P>0.05)。相关性分析结果显示TNF-α、IL-1β与ESR、CRP水平及DAS28评分呈正相关(P<0.05),IL-10与ESR、CRP水平及DAS28评分无明显相关。结论:RA患者血浆中TNF-α、IL-1β及IL-10可作为疾病活动度的重要参考指标。

Objective ： To investigate rheumatoid arthritis （RA） patients＇ plasma levels of TNF - α, IL - 1β and IL - 10 and explore the correlation among TNF - α, IL - 1 β, IL - 10 and levels of ESR and CRP which were RA disease activity indices and DAS28 scores. Methods ： rheumatoid arthritis ; TNF - α ; IL - 1β ; IL - 10 ; correlationThe relevant clinical data of a total of 30 patients with RA and 10 healthy volunteers were collected, detecting C - reactive protein （CRP） ,erythrocyte sedimentation rate （ESR） and DAS28 score. ELISA method was used to detect the plasma levels of TNF - α, IL - 1β and IL - 10. Analyze the correlation between inflammatory cytokines and disease activity indices. Results ： The plasma levels of TNF - α and IL - 1β in patients with active RA group were significantly higher than those of the control group （ P 〈 0.05 ） while IL- 10 levels were significantly decreased （ P 〈 0.05 ）. Compared with the healthy control group, the levels of TNF - α and IL - 10 in RA remission group were significantly increased （ P 〈 0.05 ） and IL - 1β had no significant change （ P 〉 0.05）. Correlation analysis showed that TNF - α, IL - 1β and levels of ESR and CRP and DAS28 scores were positively correlated （ P 〈 0.05 ）. IL - 10 and levels of ESR and CRP and DAS28 score had no significant correlation. Conclusion： RA patients＇ plasma levels of TNF- α, IL- 1β and IL- 1β can be used as important reference indices for disease activity.
