摘要
目的探讨血小板/淋巴细胞计数比值(PLR)与缺血性脑血管病(ICVD)的相关性。方法选择2015年1月至2016年1月在该院就诊的307例ICVD患者,其中脑梗死263例,短暂性脑缺血发作(TIA)44例;另外收集同期健康体检者307例作为健康对照组。记录两组年龄、性别、吸烟史、饮酒史等,并记录淋巴细胞计数、血小板计数,计算PLR值等血常规检查指标。比较两组PLR值,并运用逻辑回归分析方法分析PLR值对ICVD颈动脉粥样硬化的作用。结果 ICVD患者与健康对照组相比,PLR值明显升高,差异有统计学意义(P<0.05)。ICVD组男、女性患者PLR值均高于健康对照组男、女性,脑梗死组患者PLR值均显著高于健康对照组,差异均有统计学意义(P<0.05)。307例ICVD患者中,68例颈动脉内-中膜厚度(IMT)正常,48例颈动脉IMT增厚,191例斑块形成,其中斑块形成患者PLR值显著高于颈动脉IMT正常者、IMT增厚者,差异均有统计学意义(P<0.05)。多元logistic回归分析显示,PLR是颈动脉粥样斑块形成的独立危险因素(OR=1.012,95%CI 1.001~1.024)。结论 PLR值升高是发生ICVD的重要危险因素;PLR值与ICVD患者颈动脉粥样硬化的形成及发展密切相关,可反映其颈动脉粥样硬化程度。
Objective To investigate the correlation between platelet/lymphocyte count ratio(PLR) and ischemic cerebrovascular disease(ICVD). Methods We selected 307 cases patients of ICVD in our hosipital from January 2015 to January2016. Among them,cerebral infarction 263 cases,transient ischemic attack(TIA) 44 cases.Selecting 307 healthy cases of the corresponding period as control group.Recording age, gender, history of smoking, history of drinking of two groups,and recording the lymphocyte count, platelet count,calculating PLR value. We compared PLR value of two groups, and analyses the correlation between PLR and ICVD carotid atherosclerosis by using logistic regression analysis. Results The PLR value of the ICVD group was higher than that of the control group,the difference was statistically significan(P〈0.05).The PLR value in both male and female were higher in the ICVD group than that of male and female in the control group.The PLR value in both cerebral infarction was higher than that of the control group, the difference was statistically significan(P〈0.05). 307 cases ICVD patients,68 cases carotid IMT were normal, 48 cases carotid IMT were thickening, 191 cases plaque formation,among them,the PLR value in the plaque formation group was significantly higher than both thickening IMT and the normal IMT group, the difference was statistically significan(P〈0.05).Multivariate logistic regression analysis showed that the increased PLR value was an independent risk factor for carotid atherosclerotic plaque formation(OR=1.012,95%CI 1.001~1.024).Conclusion The PLR value is an important risk factor of ICVD.The PLR value is closely related to the formation and development of carotid atheroscleroticplaque formation in patients with ICVD,can reflect the degree of carotid artery atherosclerosis.
