来氟米特、环磷酰胺联合糖皮质激素治疗系统性红斑狼疮肾炎的临床研究

Clinical Study of Leflunomide and Cyclophosphamide Combined Glucocorticoid in the Treatment of Patients with Systemic Lupus Erythematosus

目的:探讨来氟米特、环磷酰胺联合糖皮质激素治疗系统性红斑狼疮肾炎的疗效及安全性。方法:将某院收治的82例系统性红斑狼疮肾炎患者随机分为研究组和对照组,每组41例。对照组采用环磷酰胺联合糖皮质激素强的松治疗,研究组采用来氟米特联合强的松治疗,比较两组患者治疗前后各生化及免疫指标,临床疗效及不良反应。结果:治疗后研究组患者白细胞、尿蛋白、白蛋白、ESR及补体C3等指标较治疗前均有明显改善,且改善程度与对照组比较差异显著(P<0.05);研究组总治疗有效率为95.1%,不良反应发生率为4.9%,临床疗效及不良反应发生率均优于对照组(P<0.05)。结论:来氟米特联合糖皮质激素治疗系统性红斑狼疮肾炎临床疗效显著,不良反应少,具有一定的临床使用价值。

Objective：To explore the efficacy and safety of leflunomide and cyclophosphamide combined glucocorticoid in the treatment of patients with systemic lupus erythematosus.Methods：82patients with systemic lupus nephritis treated in a hospital were randomly divided into study group and control group,with 41 cases in each group.The control group were given cyclophosphamide combined glucocorticoid prednisone therapy,and the study group were given leflunomide combined prednisone therapy,then the biochemical and immune indexes before and after treatment,clinical efficacy and adverse reactions of two groups were compared.Results：After treatment,the white blood cells,urine protein,albumin,ESR and complement C3 of study group were significantly improved compared with before treatment,and the improvement compared with the control group had significant difference（P〈0.05）;The total treatment effective rate and the incidence of adverse reactions of study group were 95.1% and 4.9%,which were significantly better than those of control group（P〈0.05）.Conclusion：The clinical effect of leflunomide combined glucocorticoid for patients with systemic lupus erythematosus is distinct,with low adverse reaction rate,which has a certain clinical value.
