摘要 目的分析表面增强激光解析电离飞行时间质谱法(SELDI-TOF-MS技术)在良恶性胸腔积液鉴别中的应用价值。方法运用SELDI-TOF-MS技术分析已诊断明确的36例恶性胸腔积液和44例良性胸腔积液,运用Biomarker Pattem(BPS)软件分析组间差异蛋白。并通过发现的高表达差异蛋白对50例胸腔积液样本进行盲筛验证,评估其临床运用价值。结果恶性胸腔积液与良性胸腔积液间存在11个差异蛋白波峰,恶性胸腔积液中高表达9个,低表达2个。选用其中质核比2768.14和7346.68的差异蛋白为标志物建立决策树模型,经BPS软件分析,诊断敏感度为90.91%,特异度为96.43%,Youden指数为0.87。结论 SELDI-TOF-MS技术可用于鉴别良恶性胸腔积液,具有较大临床运用价值。 Objective To evaluate the application value of Surface Enhanced Laser Desorption/Ionization-time of Flight-mass Spectrometry(SELDI-TOF-MS) in the identification of benign and malignant pleural effusion.Methods SELDI-TOF-MS technology was used to detect the protein mass spectrum in malignant pleural effusion of 36 patients and benign pleural effusion of 44 patients.The different protein markers were analyzed by Biomarker Pattern Software(BPS) and the initial diagnosis models were set up.The diagnosis models were verified further by blind screen to confirm the efficacy of diagnosis.Results Eleven different protein peaks were detected between benign and malignant pleural effusion.The protein peaks with a mass/charge ratio(M/Z) of 2768.14 and 7346.68 were selected to establish the classification tree model.The sensitivity of diagnosis was 90.91%,the specificity was96.43% and Youden's index was 0.87.Conclusions The SELDI-TOF-MS technology can be used to differentiate benign and malignant pleural effusion,which has great value in clinical application.
