经阴道彩色多普勒超声诊断PCOS的诊断价值

Diagnostic Value of Transvaginal Color Doppler Ultrasound in the Diagnosis of Patients with PCOS

目的:探讨经阴道彩色多普勒超声诊断多囊性卵巢综合征(PCOS)的诊断价值。方法:回顾性分析2014年11月-2016年11月本院根据临床症状及性激素水平诊断为PCOS患者71例(PCOS组),以及71例正常妇女(正常组),再根据体重指数将PCOS患者分为肥胖组(BMI≥25 kg/m^2)25例及非肥胖组(BMI<25 kg/m^2)46例,利用经阴道彩色多普勒超声测量而得两组妇女多普勒超声特征、多普勒参数,以及实验室检查LH、FSH、T,而后推算LH/FSH。结果:PCOS组患者LH、LH/FSH、T及BMI均明显高于正常组,比较差异均有统计学意义(P<0.05);PCOS组患者多普勒参数PI及RI明显低于正常组,PSV明显高于正常组,髓质内粗条状血流显示率高于正常组,平均卵巢体积大于正常组,平均卵泡数目多于正常组,比较差异均有统计学意义(P<0.05);PCOS组患者中肥胖组与非肥胖组多普勒超声参数(PI、RI、PSV)、卵泡数目、粗条状血流及卵巢体积比较差异均无统计学意义(P>0.05)。结论:经阴道超声PCOS的诊断有较好的临床参考应用价值。

Objective： To observe the diagnostic value of transvaginal color doppler ultrasound in diagnosis of patients with PCOS.Method： 71 women with PCOS were selected from November 2014 to November 2016 according to the clinical symptoms and sex hormone levels as the PCOS group, another 71 normal women were selected as the normal group.According to body mass index （ BMI ）, patients in PCOS group were divided into obese group （ BMI≥25 kg/m2, n=25 ） and non-obese group （ BMI〈25 kg/m2, n=46） .The Doppler sonographic features and Doppler parameters of two groups were given transvaginal color Doppler ultrasound measurement, the value of LH, FSH and T were given laboratory examination, then calculates LH/FSH.Result： The value of LH, LH/FSH, T and BMI of the PCOS group were higher than the normal group, the differences were statistically significant （ P〈0.05 ） .In PCOS group, the Doppler parameters of PI and RI were lower than normal group, PSV was higher than the normal group, the medulla thick strip rate of blood flow was higher than the normal group, the average ovarian volume and the average number of follicles were higher than the normal group, the differences were statistically significant （ P〈0.05 ） .PCOS patients in obese and non-obese group Doppler ultrasound parameters （ PI, RI and PSV ）, the number of follicles, thick strips and ovarian blood flow volume differences had no statistical significance （P〉0.05） .Conclusion： Transvaginal sonographic diagnosis of PCOS has better clinical reference application value.
