妊娠期高血压疾病患者超声心动图检查的临床意义

Value of echocardiography in hypertensive disorder complicating pregnancy

目的探讨超声心动图在妊娠期高血压疾病患者中的应用价值。方法回顾性分析在我院产科妊娠期高血压疾病患者94例的超声心动图检查及临床资料。结果妊娠期高血压疾病患者左心结构及功能的改变,左心腔扩大、左室壁增厚、心包积液、左室收缩、舒张功能减低等,与正常妊娠组比较,差异具有统计学意义(P<0.05);左室壁增厚的患者病情重,剖宫产、产后出血、宫内窘迫、死胎、早产及新生儿窒息等发生率均明显升高。结论超声心动图不仅可以观察妊娠期高血压疾病患者左心结构和功能的改变,还可以据此评估该类患者产前病情、预测产后出血及新生儿不良事件发生率,指导临床决策,可作为该类孕妇的产前常规检查。

Objective To investigate the value of echocardiography in hypertensive disorder complicating pregnancy（HDCP）.Methods A retrospective analysis of echocardiography and clinical data of 94 inpatients with HDCP,from June 2013 to June 2015 in department of obstetrics in our hospital.Results Changes in left ventricular structure and function of the patients with HDCP included left atrial enlargement,left ventricular enlargement,left ventricular hypertrophy（LVH）,pericardial effusion（PE）,left ventricular systolic or diastolic function failure,there were significant differences compared with normal pregnancy group（P〈0.05）.The patients with LVH,were severe and the rate of cesarean section,postpartum hemorrhage,stillbirth,premature birth,endouterine asphyxia and neonatal asphyxia were significantly increased.Conclusion Echocardiography not only can observe the changes of left ventricular structure and function of patients with HDCP,but also can assess the prenatal patient,and predict incidence of postpartum hemorrhage and neonatal adverse events,to guide the clinical decision,which can be used as the prenatal routine examination of pregnant women.
