综合护理干预与社区慢病管理对临终患者生存质量影响的研究

Influence of comprehensive nursing intervention and community chronic disease management on the quality of life of terminal patients

目的探讨施综合护理干预和社区慢病管理对临终病人生存质量的影响。方法对76例临终患者和家属进行饮食宣教、用药宣教、生活护理及症状护理等综合干预,以及行为评估、风险预测、心理支持等慢病管理措施,实施干预1~3个月后,分别测定患者生存质量和心理焦虑状况。结果实施综合护理干预与社区慢病管理措施1个月和3个月后分别与干预前比较,临终患者生存质量的社会功能、心理功能、躯体功能、物质功能维度提高,心理焦虑评分降低,差异均有统计学意义(P<0.05)。结论综合护理干预与社区慢病管理是对患者临终关怀的有效措施和方法。

Objective To explore the influence of comprehensive nursing intervention and community chronic disease management on the quality of life of terminal patients.Methods Education on diet and medications was offered for the 76 terminal patients.Daily nursing and symptom nursing care were provided.Besides some chronic diseases management measures such as behavior assessment,risk prediction and psychological supports were implemented,too.One to three months after the beginning of the intervention,the quality of life of the patients and the anxiety were assessed.Results The scores for the life quality dimensions of the social function,psychological function,physical function and material function one month after the intervention and those of three months after the intervention were higher than those before the inventions,and the scores for the anxiety were lower,with the differences being statistically significant（P〈0.05）.Conclusion Comprehensive nursing intervention and community chronic diseases management is an effective way of terminal care.
