血浆致动脉粥样硬化指数与急性冠脉综合征相关性研究

Study on Correlation between AIP and ACS

目的研究血浆致动脉粥样硬化指数(AIP)与急性冠脉综合征(ACS)的关系,为临床ACS鉴别诊断提供新的思路。方法纳入2015年1月—2016年1月我科收治的疑似ACS患者132例,所有患者均行冠脉造影检查,并根据检查结果将这些患者分为ACS组和对照组,比较两组临床资料和生化指标。结果 ACS患者85例(64.4%),对照组患者47例(35.6%)。与对照组相比,ACS组患者年龄大、糖尿病患者多、总胆固醇(TC)和AIP升高(P<0.05),白细胞总数、中性粒细胞数、空腹血糖和血肌酐均显著升高(P<0.01),高密度脂蛋白胆固醇(HDL-C)显著降低(P<0.01);而性别、体重指数(BMI)、吸烟、高血压病史、收缩压、舒张压、甘油三酯(TG)和低密度脂蛋白胆固醇(LDL-C)无显著差异。多因素Logistic回归分析发现AIP与ACS相关性最大(OR=3.421,P<0.01)。在ACS亚组中,年龄、性别、高血压、糖尿病患者、BMI、收缩压、舒张压、空腹血糖、血肌酐、TC、TG、HDL-C、LDL-C和AIP在不稳定型心绞痛(UA)、非ST段抬高性心肌梗死(NSTEMI)和ST段抬高性心肌梗死(STEMI)中无显著差异(P>0.05),而吸烟史、白细胞总数和中性粒细胞数在三组中有显著差异(P<0.05)。结论 AIP与ACS密切相关,它可能成为临床胸痛鉴别诊断的指标之一。

Objective To explore the correlation between atherogenic index of plasma（AIP） and acute coronary syndrome（ACS）.Methods 132 ACS suspicious patients hospitalized during the period from Jan., 2015 to Jan., 2016 received coronary angiographic examination and based on the results, they were divided into 2 groups： ACS group（n=85, 64.4%） and control group（n=47, 35.6%）; a comparative analysis was made to the clinical data and biochemical indexes between the 2 groups. Results There were more older patients with diabetes and increased TC and AIP in ACS group than in control group（P〉0.05）; the leukocyte count, neutrophil count, the levels of fasting blood glucose and serum creatinine of the patients in ACS group increased obviously while HDL-C decreased greatly, much more than those of the patients in control group（P〉0.01）; no statistical difference in gender, BNI, smoking history, hypertension course,systolic blood pressure（SBP）, diastolic blood pressure（DBP）, TG and LDL-C was found between the 2 groups; multiple logistic regression analysis revealed a great correlation between AIP and ACS（OR=3.421, P〉0.01）.Conclusions AIP is closely correlated to ACS, it may be one differential diagnosis index of chest pain in clinic.
