两种心理社会适应量表在化疗期乳腺癌患者中的应用比较

A comparative study of two psychosocial adjustment scales for breast cancer patients receiving chemotherapy

目的比较两种心理社会适应量表在化疗期乳腺癌患者中的应用效果。方法选取化疗期乳腺癌患者743例为研究对象,分别采用疾病心理社会适应问卷(PAIS-SR)、乳腺癌患者心理社会适应问卷测量患者的心理社会适应状况,以乳腺癌患者生存质量量表(FACT-B)为效标,比较两种心理社会适应量表的相关性及效度。结果化疗期乳腺癌患者PAIS-SR得分46.77±15.89,乳腺癌患者心理社会适应问卷得分169.65±25.52,FACT-B量表得分97.47±17.92;PAIS-SR总分及各维度与心理社会适应问卷总分具有显著相关性(r值为-0.245^-0.668,均P<0.01);乳腺癌患者心理社会适应问卷总分、PAIS-SR总分与FACT-B得分的相关系数分别为0.730、-0.780(均P<0.01)。结论两种问卷所测患者心理社会适应度均处于中等水平,两种问卷的相关性及与效标量表的相关性较强,均可用于化疗期乳腺癌患者的心理社会适应状况的测量。PAIS-SR量表测量的维度更全面,心理社会适应问卷更偏重于患者心理状况的测量,临床应用中可根据实际需要选择合适的量表。

Objective To compare the effects of two psychosocial adjustment scales in breast cancer patients receiving chemotherapy. Methods A total of 743 breast cancer patients receiving chemotherapy were invited to fill out the Self-report Psychosocial Adjust ment to Illness Scale （PAIS-SR）, the Psychosocial Adaptation Questionnaire-Breast Cancer （PAQBC） and Functional Assessment of Cancer Therapy-Breast （FACT-B） scale. The criterion related validity of the two adjustment scales was supported by the FACT-B. Results The respondents scored 46.77±15.89 on PAIS-SR, 169.65±25.52 on PAQ-BC, and 97.47±17.92 on FACT- B. The total PAIS-SR score and subscale scores were significantly correlated with the total PAQ-BC score （r=- 0. 245- 0. 668, P〈0.01 for all）. Using the FACT-B as criterion, the correlation coefficients of PAIS-SR and PAQ-BC were 0. 730 and 0. 780 respectively （P〈0.01 for both）. Conclusion Psychosocial adjustment of breast cancer patients measured by the PAIS SR and PAQ BC was at moderate level. Both scales can be used in breast cancer patients receiving chemotherapy for they show high criterion-re lated validity with the FACT-B. The PAIS-SR is more comprehensive, while the PAQ-BC focuses on mental state, thus indicating actual situation should be considered when measuring psychosociad adjustment of patients.
