摘要
目的探讨PDCA应用于新生儿重症监护病房(NICU)新生儿留置静脉留置针非预期拔管的效果。方法选取2013年1月-2016年1月NICU行静脉留置针穿刺的新生儿300例,按随机数字表法将其分为常规组178例和PDCA组122例。常规组进行静脉留置针常规维护,PDCA组根据PDCA循环理论进行静脉留置针维护。比较2组静脉留置针保留时间、非预期拔管率、静脉炎发生率。结果 PDCA组留置针保留时间为(96.44±18.72)h,长于常规组的(72.25±13.68)h,2组比较,差异有统计学意义(t=12.928,P<0.001);非预期拔管率、静脉炎发生率低于常规组(χ~2=4.236,P=0.040;χ~2=4.175,P=0.041)。结论 PDCA应用于NICU新生儿静脉留置针维护中,可延长留置针保留时间,减少非预期拔管和静脉炎的发生。
Objective To explore the effect of applying PDCA in reducing unexpected needle extraction in neonatal intensive care unit (NICU). Methods A total of 300 newborns with venous indwelling needles in NICU department of our hospital between January 2013 and January 2016 were selected and divided into a PDCA group ( n= 122) and a normal group (n = 178) according to their parents' choice. The normal group received routine nursing for the peripheral venous indwellingneedles, while the PDCA group was given nursing according to the PDCA cycle theory. heretention time of indwelling needles, unexpected needle extraction rate and the incidence rate ofphlebitis were compared between the two groups. Result The retention time of indwelling needlesof PDCA group was (96. 44± 18. 72) h,significantly longer than (72. 25± 13. 68)h of the normal group t = 12.928, P 〈0. 001). The unexpected needle extraction rate and phlebitis incidence rate of the PDCA were lower than the normal group (χ2 = 4 .236, P = 0.040; χ2 =4. 175, P =0.041). Conclusion PDCA circulation nursing can increase the retention time of indwelling needles and lower unexpected needle extraction and phlebitis among newborns in the NICU department.
