不同手术切口治疗胫腓骨中下段双骨折的临床疗效对比观察

Clinical Observation of Different Surgical Incision for the Treatment of Double Fractures of the Middle and Distal Part of Tibia and Fibula

目的:观察比较不同手术切口治疗胫腓骨中下段双骨折的临床效果。方法:入组病例为2013年3月~2016年3月到笔者所在医院实施诊治的胫腓骨中下段双骨折患者,共计病例数74例,基于患者和其家属的知晓同意下,按照平均分组原则将其划分成两组,分别为试验组与对照组,每组37例,前组患者行小腿前外侧单切口切开复位钢板内固定术,后组行小腿内外侧双切口切开复位钢内固定术治疗,观察两组患者治疗效果,记录二者切口愈合时间、并发症、出血量、骨痂形成时间、手术时间等情况。并予以对比分析。结果:实验组治疗总有效率明显优于对照组患者,差异有统计学意义(P<0.05)。结论:在胫腓骨中下段双骨折的临床治疗中以小腿前外侧单切口实施切开复位钢板内固定术,优势明显,安全可靠,可行性强。

Objective：To observe and compare the clinical effect of different surgical incision in the treatment of double fractures of the distal tibia and fibula Methods：74 cases of patients with tibia and fibula were selected ftom March2013 to March 2016 to the hospital diagnosis and treatment in patients with fractures,based on the awareness of patients and their families agree,in accordance with the principle of the average packet to be divided into two groups,respectively as experimental group and control group,each group of 37 cases.Before the group underwent anterolateral incision open reduction and plate fixation,reduction of steel fixation for both medial and lateral incisions group,the therapeutic effect of two groups were recorded,two operation time,callus formation time,bleeding volume,complications and wound healing time were compared and analyzed.Results：In experimental group,the total efficiency of treatment was significantly higher than the control group,the incidence rate was significantly lower than the control group and the postoperative complications between groups of data were statistically significant,P〈0.05.Conclusion：Open reduction and internal fixation with single incision in the anterior lateral leg in the clinical treatment of the middle and lower part of the tibia and fibula,the advantage is obvious,safe and reliable,and the feasibility is strong.
