初治痰涂片阳性肺结核单发空洞的影像观察对疗效及预后评估作用

Evaluation of Dynamic Imaging of Initial Smear Positive Pulmonary Tuberculosis Single Cavity Evolution to Observe the Curative Effect and Prognosis Assessment

目的:研究肺结核初治痰涂阳者单发空洞影像改变对预后的影响。方法:回顾分析我院自2012年9月~2015年9月以来,于我科治疗的197例肺结核初治痰涂阳者临床资料,依据患者治疗后单发空洞的影像改变分为五组,对比分析各组治疗前后的X线影像学特征及动态变化规律,观察各组影像学变化下血沉、C-反应蛋白、IFN-γ水平的改变情况。结果:各组治疗前主要影像学特征为厚壁空洞或呈厚薄不均表现,也可伴有壁内结节,类似于肺癌空洞。结论:肺结核单发空洞转归具有一定的规律性,对肺结核初治痰涂阳者抗结核治疗阶段观察结核空洞的动态变化,有助于评估预后。

Objective：To study the effect of single cavity image change on the prognosis of patients with primary sputum smear positive in patients with pulmonary tuberculosis.Methods：A retrospective analysis of our hospital since September September 2012 to 2015,in our hospital treatment of 197 cases of pulmonary tuberculosis initial treatment smear positive were clinical data,basis after treatment in patients with single hole image changes were divided into five groups,comparative analysis before and after the treatment group X-ray imaging features and dynamic changes were observed each image changes of erythrocyte sedimentation Tate,C-reactive protein and IFN-levels under change.Results：Among groups before treatment imaging characteristics for thick wall cavities or is uneven performance,accompanied by wall nodules,similar to lung cancer cavity.Conclusion：The prognosis of solitary pulmonary tuberculosis cavity has a certain regularity,the new smear positive pulmonary tuberculosis anti tuberculosis treatment phase dynamic observation of tuberculous cavity,are helpful to evaluate the prognosis.
