摘要
目的:探讨超声刀在食管癌左胸一切口根治术(Sweet术)中的应用价值。方法:2011年1月至2012年6月使用超声刀切除食管中下段鳞癌28例,对照组20例,比较两组在手术时间、术中出血量、淋巴结清扫数目及术后72h引流量的差异。结果:超声刀组和对照组手术时间分别为(166±24)min和(206±32)min;术中出血量分别为(283±59)ml和(310±47)ml;术后72h引流量分别为(989±214)ml和(1 203±403)ml;术中淋巴结清扫数分别为(19.5±7)枚和(16.5±6)枚;两组比较总手术时间和术后72h引流量差异有统计学意义(P<0.05);术中出血量及淋巴结清扫数目两组比较无统计学意义。结论:超声刀可缩短食管Sweet手术时间,减少术后的胸腔引流,有助于淋巴结的清扫。
Objective: To investigate the safety and feasibility of ultrasonic scalpel in the Sweet operation for esophageal carcinoma.Methods: Totally 48 patients with middle and lower parts esophageal carcinoma were divided into two groups: Ultrasonic scalpel group(28 cases) and control group(20 cases).The operation time,bleeding volume,the number of lymph nodes and the volume of postoperative thoracic drainage were compared.Results: The average operation time and postoperative 72 h quantity of thoracic drainage in the study group were significantly lower than those in the control group(p〈0.05).There was no significant difference between the two groups in the volume of bleeding and the number of lymph nodes(p〉0.05).Conclusion: Ultrasonic scalpel can be safely and reliably used to the Sweet operation for esophageal carcinoma with less operation time and less postoperative thoracic drainage.
