江苏省某三级乙等医院病毒性肝炎患者住院费用及影响因素分析

Analysis on hospitalization expenses and influencing factors of patients with viral hepatitis in a Grade-3 Level B Hospital of Jiangsu Province

目的了解江苏省病毒性肝炎患者住院费用,探究影响因素,为合理控制其住院费用、切实缓解患者的经济负担提供参考。方法收集江苏省某三级乙等医院2013年10月—2016年4月因病毒性肝炎及其并发症收治住院患者的住院信息,对病例的住院费用特点及其影响因素进行统计分析。结果共收集病毒性肝炎患者778例,男性561例(72.11%),女性217例(27.89%),平均年龄为(41.79±19.02)周岁;病毒性肝炎患者在病原学的5个分型上以乙型肝炎(70.23%)和戊型肝炎(21.84%)为主;甲型肝炎的住院天数最短(5.5 d),戊型肝炎的住院天数最长(16 d);次均住院费用为7 063.47元,其中戊型肝炎费用最高(10 582.78元),其次为丙型肝炎(9 272.37元)、乙型肝炎(7 829.61元),费用最低的为甲型肝炎(2 294.19元);分析结果显示,性别、年龄、支付方式、病原学分型、是否合并其他诊断、住院天数及出院情况是住院费用的影响因素。结论合理缩短住院天数,进一步加大针对老年群体的社会基本医疗保险投入,有利于缓解病毒性肝炎患者的经济负担。

[Objective]To understand the hospitalization expenses of patients with viral hepatitis in Jiangsu Province,explore the influencing factors,and provide the reference for reasonably controlling the hospitalization expenses and effectively alleviating the economic burden of patients.[Methods]The information of viral hepatitis and complications patients who were treated in a Grade-3Level B Hospital of Jiangsu Province from October 2013 to April 2016 were collected,to statistically analyze the characteristics of hospitalization expenses and the influencing factors.[Results]Totally 778 cases of viral hepatitis were collected,including 561（72.11%）male cases and 217（27.89%）female cases,and the average age was（41.79±19.02）years old. For five etiology types of viral hepatitis,hepatitis B and hepatitis E accounted for 70.23% and 21.84% respectively. The hospitalization stay of hepatitis A patients was the shortest（5.5 days）,while that of hepatitis E patients were the longest（16 days）. The average expense was 7 063.47 Yuan per hospitalization,and the expense of hepatitis E patients was the highest（10 582.78 Yuan）,followed by hepatitis C patients（9 272.37 Yuan）and hepatitis B patients（7 829.61 Yuan）,and the expense of hepatitis A patients was the lowest（2 294.19Yuan）. The analysis results showed that sex, age, payment method, etiological type, combined with other diagnosis,hospitalization stay and discharge status were the influencing factors of hospitalization expenses.[Conclusion]Reasonably shortening the hospitalization stay and further enlarging investment of social basic medical insurance for elderly population may help to alleviate the economic burden of patients with viral hepatitis.
