儿童哮喘支气管肺泡灌洗液细胞学计数及白细胞介素-8变化

Changes of cytological count and interleukin-8 in bronchoalveolar lavage fluid in children with asthma

目的探讨支气管哮喘BALF细胞学计数及白细胞介素-8(IL-8)变化。方法选择2015年5月-2016年5月住院哮喘急性发作患儿60例为哮喘组,另选取同期行支气管镜下异物取出术患儿60例为对照组,对2组BALF中细胞学计数和IL-8水平进行检测,比较不同哮喘程度肺泡灌洗液中细胞学计数和IL-8水平变化。结果 2组BALF中有核细胞计数差异无统计学意义(Z=-1.44,P>0.05);2组BALF中EOS比例差异有统计学意义(P<0.01)。随着哮喘发作程度加重,BALF中EOS计数显著升高;BALF中IL-8浓度显著升高,差异均有统计学意义(P<0.01);BALF中EOS计数和IL-8水平与哮喘发作程度呈正相关(r=0.87、0.94,P<0.01);结论 BALF中EOS计数和IL-8浓度能反映哮喘气道炎症,对指导哮喘的诊断和严重程度有一定参考价值。

Objective To observe the changes of cytological count and IL-8 in bronchoalveolar lavage fluid in asthma.Methods 60 children with acute episode of asthma hospitalized from May 2015 to May 2016 were selected as the asthma group,and60 children with foreign body taken out by bronchoscopy were selected as the control group.The cytological count and IL-8levels in bronchoalveolar lavage fluid were detected and compared in both groups.Results The nuclear cell counts in bronchoalveolar lavage fluid between the 2 groups were compared,and there was no statistical significance on the differences between them（Z =-1.44,P〉0.05）.But the differences of eosinophils ratio in bronchoalveolar lavage fluid between the 2 groups was statistically significant（P〈0.01）.With the exacerbation of asthma,the counting of eosinophils in bronchoalveolar lavage fluid significantly increased,while the concentration of IL-8 in bronchoalveolar lavage fluid significantly increased,with the differences statistically significant（P〈0.01）.The counting of eosinophils and the IL-8 levels in bronchoalveolar lavage fluid were positively correlated with the degree of bronchial asthma（r = 0.87,0.94,P〈0.01）.Conclusion The counting of eosinophils and the IL-8 levels in bronchoalveolar lavage fluid can reflect airway inflammation,which has a certain reference value to guide the diagnosis and severity of asthma.
