平均血小板体积、纤维蛋白原及血栓弹力图对成年人突发性耳聋诊断的价值

Value of mean platelet volume,fibrinogen and thrombelastogram in the diagnosis of sudden sensorineural hearing loss in adults

目的探讨MPV、Fg及TEG在成年人突发性耳聋中的诊断价值。方法收集成年突发性耳聋患者110例作为突发性耳聋组,70例同期健康体检者作为对照组,检测各组MPV、Fg和TEG指标,做相关性分析,并评估MPV和Fg对突发性耳聋的诊断价值。结果突发性耳聋组MPV、Fg及Ma值高于对照组,R值和K值则低于对照组,差异有统计学意义(P<0.05)。突发性耳聋组中,听力损伤程度越重,MPV、Fg及Ma值越高,R值和K值越低;相关性分析显示,MPV和Fg均与Ma值呈正相关,同时与K值和R值呈负相关。ROC曲线分析显示,MPV诊断突发性耳聋的敏感度为89.1%,特异度为75.0%;Fg诊断突发性耳聋的敏感度为73.6%,特异度为70.0%。结论 MPV、Fg及TEG指标的检测对成年人突发性耳聋的诊断具有一定的价值。

Objective To investigate the significance of mean platelet volume（MPV）,fibrinogen（Fg） and thrombelastogram（TEG） index in the diagnosis of the adult sudden sensorineural hearing loss（SSHL）.Methods 110 adults suffering from SSHL were enrolled as SSHL group.70 healthy subjects were enrolled as the control group.MPV,Fg and TEG index were detected in all the groups.The correlations among the indexes in the SSHL group were analyzed.The diagnostic performance of MPV and Fg for SSHL were analyzed.Results MPV,Fg and Ma value in SSHL group were significantly higher than those in control group,while R value and K value were significantly lower than the control group,with the difference statistically significant（P〈0.05）.In SSHL group,the heavier the hearing loss degree was,the higher MPV,Fg and Ma value were,while the lower R value and K value were; MPV and Fg both had positive correlation with Ma,while had negative correlation with R value and K value.ROC curves show that the diagnostic sensitivity and specificity of MPV for SSHL were respectively 89.1% and75.0%,and the diagnostic sensitivity and specificity of Fg for SSHL were respectively 73.6% and 70.0%.Conclusion The determinations of MPV,Fg and TEG index play a significant role in the diagnose of the adult SSHL.
