摘要
目的探讨口腔癌患者术后早期外周血免疫功能指标的变化。方法选取2014年6月至2016年6月间山东中医药大学第二附属医院收治的70例口腔癌患者,分别于手术前后抽取患者外周静脉血,对外周血中免疫功能指标CD3^+、CD3^+CD4^+、CD3^+CD8^+、CD4^+/CD8^+、T+B+NK细胞、CD19+和CD16+/CD56+进行检测,另选取同期我院体检的70例健康人员外周静脉血免疫功能指标检测结果作为正常参考值。结果口腔癌患者术后1周的免疫功能指标与正常值比较:CD3^+CD8^+升高,CD4^+/CD8^+降低,差异均有统计学意义(均P<0.05)。与术前1周比较,口腔癌患者术后1周CD3^+CD8^+降低,术后2周CD3^+CD8^+升高,但仍低于术前1周水平,差异均有统计学意义(均P<0.05)。与术前1周比较,口腔癌患者术后1周和术后2周的CD4^+/CD8^+均升高,差异均有统计学意义(均P<0.05)。与术前1周比较,口腔癌患者术后2周T+B+NK细胞较低,差异有统计学意义(P<0.05)。口腔癌患者术后1周CD19+与术前1周比较升高,术后2周与术后1周比较较低,但仍高于术前1周,差异有统计学意义(P<0.05)。与术前1周比较。口腔癌患者术后1周和术后2周CD16+/CD56+均较低,差异均有统计学意义(均P<0.05)。结论口腔癌患者免疫功能受到一定程度的抑制,通过手术治疗能够有效缓解细胞免疫抑制作用。
Objective To explore changes in early postoperative peripheral immune function in patients with oral cancer. Methods Seventy oral cancer patients treated at The Second Affiliated Hospital of Shandong University were selected from June 2014 to June 2016. Peripheral venous blood was collected before and after operation to test CD3~+,CD3~+CD4~+,CD3~+CD8~+,CD4~+/ CD8~+,T + B + NK,CD19+and CD16+/ CD56+. Peripheral venous blood test results of another 70 healthy controls were set as a reference for immune function. Results At 1 week after the surgery,CD3~+CD8~+increased and CD4~+/ CD8~+reduced compared with the normal reference value( P 0. 05). Compared with 1 week before the surgery,CD3~+CD8~+decreased at 1 week after the surgery and increased at 2 weeks after the surgery but was still below the level at 1 week before the surgery( P 0. 05). Compared with 1 week before the surgery,CD4~+/CD8~+increased at both 1 week and 2 weeks after the surgery( P 0. 05). Compared with 1 week before the surgery,T + B + NK level was lower at postoperative 1 week( P 0. 05). At 1 week after the surgery,CD19+increased compared with preoperative 1 week,and was lower at 2 weeks postoperatively than postoperative 1 week,but was still higher than that of preoperative 1 week( P 0. 05). Compared with 1 week before the surgery,CD16+/ CD56+was lower at both 1 week and 2 weeks postoperatively( P 0. 05). Conclusion Immune function was inhibited in a certain degree in patients with oral cavity cancer,which can be effectively alleviated by surgical treatment.
