基于现代名老中医经验的肺脓肿病因病机及证素规律研究

Study on the Etiology and Pathogenesis and Rules of Syndrome Elements on Pulmonary Abscess Disease based on the Experience of Famous Veteran Doctors of TCM

目的基于现代百余年名老中医诊治肺脓肿的文献记载,分析肺脓肿的病因病机与证素分布规律,为丰富肺脓肿病因病机提供依据。方法基于已建立的《现代名老中医肺病数据库》和《期刊中医肺病数据库》,建立《现代名老中医诊治肺脓肿文献研究数据库》,利用SPSS19.0软件进行统计分析。结果肺脓肿常见病因有热邪、内因、外邪、风邪、疫毒等。病机以痰热壅肺、热毒壅肺、肺热炽盛、肺络损伤、热壅血瘀较为常见。13个证素中,病性证素以热、痰、毒、阴虚、血瘀较常见;病位证素以肺为主,涉及脾、胃、肝、肾。证素组合以两证素和三证素组合为主。病性证素组合以痰+热、热+毒、热+血瘀较为常见,病位均主要在肺。结论现代名老中医对肺脓肿病因病机的理论认识除与现行的教材、专著论述大致相同外,尚有饮食所伤、吸烟的病因与气阴两虚、肺脾两虚的病机,需要进一步总结。

Objective To explore the rules of etiology and pathogenesis distribution, syndrome elements distribution and combination in treating pulmonary abscess disease through studying the experience of modern famous doctors of TCM. Methods Literatures which met the inclusion criteria were retrieved from the existing Lung Disease Database of Modern Famous Doctors of Chinese Medicine and Lung Disease Database of Journals for the establishment of Literature Research Database of Experience of Modern Famous Doctors of Chinese Medicine in Treating Pulmonary Abscess Disease. The data were studied by using SPSS19.0 statistical software. Results There were 13 kinds of etiology, mainly heat evil, internal cause, external evil and wind evil. Pathogenesis included phlegm-heat ob- structing lung most frequently, followed by toxic heat, intense lung heat. The pathogenic syndrome elements of pulmonary abscess dis- ease were mainly composed of heat, phlegm, toxin and yin deficiency. The syndrome element of pathogenic location was mainly the lung. The combination rule was dominated by the combination of three syndrome elements and two syndrome elements. The combination of two syndrome elements of disease nature were sputum pathogen combined with heat pathogen, the toxin combined with heat pathogen, the heat pathogen combined with static blood. The disease location was mainly the lung. Conclusion The rules of etiology distribution, pathogenesis distribution, syndrome elements distribution and combination in treating pulmonary abscess disease by modern famous doctors of TCM are valuable and deserve further research.
