银离子敷料换药与常规普通换药在门诊慢性创面换药护理的对比研究

Comparative research of silver ion dressing change and conventional dressing change in outpatient nursing for chronic wound dressing change

目的探讨银离子敷料在治疗慢性创面换药护理中的疗效。方法 80例慢性创面患者,采用随机数字表法分为实验组和对照组,各40例。两组患者均清创止血后,进行控感染、改善微循环等常规处理,另外实验组患者使用银离子敷料直接覆盖创面或者填充伤口,对照组患者彻底清创止血后用无菌纱布及凡士林纱布填塞或覆盖创面填充固定。比较两组患者治疗效果、换药次数、愈合时间及细菌培养检出病例。结果对照组患者治疗有效率57.5%,明显低于实验组的97.5%,差异具有统计学意义(P<0.05);实验组患者的换药次数为(7.89±2.54)次、创面愈合时间为(45.20±6.25)d、细菌培养检病例数1例,对照组患者的换药次数为(14.25±2.14)次、创面愈合时间为(87.35±7.45)d、细菌培养检病例数18例,比较差异均具有统计学意义(P<0.01)。结论银离子敷料治疗慢性创面的疗效明显优于常规普通换药。

Objective To investigate curative effect by dressing change nursing of silver ion dressing in the treatment of chronic wound. Methods A total of 80 patients with chronic wound were divided by random number table into experimental group and control group, with 40 cases in each group. Both groups received conventional treatment by debridement, hemostasis, anti-infection and microcirculation improvement. The experimental group received silver ion dressing for wound coverage or fill, and the control group received sterile gauze and petrolatum gauze for wound coverage or fill after debridement and hemostasis. Comparison was made on curative effect, dressing change times, healing time and detected cases in bacterial culture between the two groups. Results The control group had treatment effective rate as 57.5%, which was obviously lower than 97.5% in the experimental group, and the difference had statistical significance（P〈0.05）. The experimental group had dressing change times as（7.89±2.54） times, wound healing time as（45.20±6.25） d and 1 detected case in bacterial culture. The control group had dressing change times as（14.25±2.14） times, wound healing time as（87.35±7.45） d and 18 detected cases in bacterial culture. Their difference had statistical significance（P〈0.01）. Conclusion Silver ion dressing provides remarkably better curative effect in treating chronic wound than conventional dressing change.
