尼莫地平治疗老年高血压性脑出血患者的效果观察

Observation on effect of nimodipine in the treatment of senile patients with hypertensive cerebral hemorrhage

目的探讨尼莫地平治疗老年高血压性脑出血患者的临床效果。方法 90例老年高血压性脑出血患者,随机分成研究组及对照组,各45例。两组均采用止血、降颅压、保持水电解质平衡等常规治疗,对照组在常规治疗的基础上加用硝普钠进行治疗,研究组在常规治疗的基础上加用尼莫地平进行治疗。比较分析两组治疗前后神经功能缺损评分(NIHSS)及临床疗效。结果研究组总有效率91.1%高于对照组73.3%,差异具有统计学意义(χ~2=4.865,P<0.05)。治疗前,两组患者NIHSS评分比较,差异无统计学意义(P>0.05);治疗3周末,两组患者NIHSS评分均较治疗前下降,且研究组患者治疗后NIHSS评分明显低于对照组,差异均具有统计学意义(P<0.05)。结论尼莫地平治疗老年高血压性脑出血患者的临床效果优于硝普钠的治疗效果,可有效改善患者脑神经功能缺损程度,值得应用。

Objective To explore the clinical effect of nimodipine in the treatment of senile patients with hypertensive cerebral hemorrhage. Methods A total of 90 senile patients with hypertensive cerebral hemorrhage were randomly divided into research group and control group, with 45 cases in each group. Both groups received hemostasis, intracraninal pressure, and maintain water and electrolyte balance and other routine treatment, and the control group also received sodium nitroprusside for treatment. The research group also received nimodipine for treatment. Comparison were made on National Institutes of Health stroke scale（NIHSS） score before and after treatment and clinical curative effect. Results The research group had higher total effective rate as 91.1% than 73.3% in the control group, and the difference had statistical significance（χ^2=4.865, P〈0.05）. There was no statistically significant difference in NIHSS score before treatment in two groups（P〉0.05）. After 3 weeks of treatment, both groups had lower NIHSS score than before treatment, and the research group had obviously lower NIHSS score after treatment than that of the control group. Their difference had statistical significance（P〈0.05）. Conclusion Nimodipine shows better clinical effect than sodium nitroprusside for the treatment of senile patients with hypertensive cerebral hemorrhage. It also can effectively improve cranial nerve function defect degree, therefor it is worthy of application.
